  • 2022 № 10 Approaches to developing an information resource for managing patient flows in a medical organization during a global biological challenge (on the example of COVID-19)

    Patient flow management in medical organizations and rational use of human, material, medical and technical resources during the period of global biological challenge are the key to the effective functioning of the health care system.
    The formation of a set of indicators and criteria that allow the organization, planning and management of hospitalizations should be carried out with using of modern information aggregators integrated with medical information systems both at the level of a single hospital and in general across a network of medical organizations in the region that provide specialized medical care in hospitals.
    Purpose of the study is to develop an information system architecture that allows assessing the level of use of the resources of a medical organization during a global biological challenge (on the example of COVID-19) and distributing patient flows depending on its current capabilities.
    Materials and methods. Based on the data on the results of hospitalization of 7576 patients with COVID-19 in the Noginsk Central District Hospital, 6 consecutive stages of developing an information system were formed, including user stratification and determining roles and levels of access to information for them, classifying the resources of a medical organization, developing digital models of databases data, standardization and mapping of work processes in the structural divisions of the hospital, compiling a list of input (accounting) and output (reporting) indicators correlated with each other through the use of directories and registers, as well as on the basis of logical links.
    Results. Digital layouts of 4 registration cards were developed (data of medical workers; data of patients, medical services provided to them and the time for their implementation; use of medical equipment and time spent on research; registration of hospitalizations in hospital departments), 10 reporting forms (on the activities of medical workers and medical services rendered by them; on the structure of hospitalized and diagnostic tests carried out by them; on the use of medical equipment; on hospitalizations and the use of hospital beds) and templates of analytical reports for making management decisions based on the data of reporting forms for 6 categories of users (for medical workers of the relevant structural units of the hospital heads of structural subdivisions, deputy heads of a medical organization and head of a medical organization, a specialist of a public authority of a constituent entity of the Russian Federation in the field of health protection).
    Conclusion: The information resource proposed for development makes it possible to carry out a current assessment of the workload of medical workers and medical equipment, the employment of the bed fund, compliance with time parameters when performing standard operating procedures in the structural divisions of the hospital and ensure operational management of patient flows.

  • 2022 № 5 Implementation of SCORE calculator into the medical information system of the primary care

    In the Russian Federation, disease prevention in primary care is a priority area of medical care for the adult population. An automated assessment of the risk of death from cardiovascular disease can be an auxiliary preventive tool if integrated intoa medical information system.
    P u r p o s e o f t h e s t u d y – to substantiate the automation of the process of assessing the risk of death from cardiovascular diseases over the next 10 years using the SCORE scale at the level of a city hospital.
    M a t e r i a l s a n d m e t h o d s . The study was carried outat the State Budgetary Medical Institution of Arkhangelsk region “Arkhangelsk City Clinical Hospital № 7”. The completeness and quality of electronic patient records were assessed. The SCORE calculator was integrated into the examination protocol for primary care physicians. The effects of the innovation on the completeness and quality of electronic patient records were assessed.
    R e s u l t s a n d d i s c u s s i o n . Among the served population aged 40 to 79 years, any information on risk factors for cardiovascular diseases was reflected in 28,7% of electronic patient records. Data on all variables used to assess the risk on the SCORE scale (gender, age, systolic blood pressure, smoking, cholesterol level) were present for 20,8% of the patients. The introduction of the SCORE automated calculator into the examination protocol of a general practitioner and a cardiologist made it possibleto increase the completeness of data on cardiovascular disease risk factors used for the risk assessment on the SCORE scale to 97,4%.
    C o n c l u s i o n s . The introduction of the SCORE automated calculator has significantly increased the completeness of data on the main risk factors for cardiovascular diseases in patients’ electronic patient records. The innovation makes it possible to reduce labor costs forestimatingthe risks, allowsmore effective identification of persons subject to dispensary observation.

  • 2022 № 4 The concept of implementing a business plan for a mobile medical clinic

    The article is devoted to the economic aspect of the concept of implementation and functioning of a mobile medical clinic (MMC).Despite the role of mobile medical clinics in providing medical assistance to the entire population, all social groups, the overall impact of mobile medical clinics has not often been the subject of research.
    The purpose of the study is to analyze the project of implementing the business plan of a mobile (mobile) medical clinic.
    Materials and methods. The presented business plan has a structure focused on the creation of mobile medical complexes on the territory of each subject of Russia. The introduction of such medical clinics is a necessary and important process within the framework of preserving the health and working capacity of the population. Important stages in the implementation of the business plan are: a legislative plan that includes the main regulations, a marketing plan that takes into account the segment of the services market and the demand for the planned price, and an organizational plan that includes an assessment of the total cost of the project and fixed costs.
    Results. In the article, the results show that the introduction of mobile medical clinics on the territory of the subjects of Russia will not only improve the state of health and improve the quality of care for people, but also improve the health of the population while reducing the growth rate of costs (within the framework of the all-Russian national strategy). In this regard, an organizational plan was developed that takes into account the fixed costs of the mobile clinic.
    Conclusion. The development of a business plan for the implementation of mobile medical clinics in the territory of each subject of Russia and the introduction of MMC programs into the healthcare system will increase the level of socio-economic well-being of the population.

    The organization of health support for people with cardiovascular diseases during a pandemic requires limiting direct contact with the patient. It is necessary to introduce/improve technologies for remote monitoring of the condition of high-risk patients with the possibility of prompt response in case of deterioration.
    The purpose of the work – a detailed study of the possibilities of implementing remote technologies for analyzing the condition of high-risk patients with cardiovascular diseases.
    Methods and materials: a detailed analysis of literature sources concerning remote monitoring of the condition of persons suffering from cardiovascular diseases and being at high risk was carried out. Particular attention is paid to studies conducted during the pandemic. The study of the state of the issue of interest was carried out by analyzing publications in the databases: Cyberleninka, PubMed, Elibrary, ScienceDirect, Springerlink.
    Results: the trends in the development of modern technologies for remote analysis of the parameters of the functioning of the cardiovascular system, including pulse rate, blood pressure, and electrocardiographic studies, were determined. Particular attention is paid to assessing the possibilities of counseling the patient through remote communication, options for transmitting data on the patient’s condition, methods of emergency communication.

  • 2022 № 2 Telerehabilitation of post-stroke patients: current trends in the Russian healthcare system

    With age, the risk of stroke increases significantly, but at the same time, there are tendencies for an increase in its prevalence in young and middle-aged people, which determines the urgency of the problem of rehabilitation treatment after stroke both in the world and in Russia. The use of digital technologies in neurorehabilitation can be very multifaceted and extensive. Telerehabilitation as an innovative direction of telemedicine is an important aspect within the framework of the national project «Healthcare» and the program «Digital Economy of the Russian Federation».
    P u r p o s e of the study is to analysis of the Russian experience of organizing and conducting telerehabilitation of post-stroke period in the context of the digital transformation of the healthcare system in Russia.
    M a t e r i a l s a n d m e t h o d s . The article used technologies for searching and selecting information using search engines (www. google.ru, www.yandex.ru), specialized and bibliographic databases (PubMed, eLIBRARY.RU), methods of analysis and synthesis, analytical data processing.
    R e s u l t s . The study showed that telerehabilitation has a high clinical value of adherence to the treatment of patients of post-stroke period and is not a separate type of medical activity, but an innovative therapeutic and prophylactic method that expands the possibilities of therapy and improves the process of interaction in the «doctor-patient» and «doctor-doctor» communication systems.
    This technology of restorative telemedicine makes it possible to increase the level of scientific knowledge, skills and abilities of healthcare organizers and medical workers in order to strengthen the national healthcare system of Russia, as well as to rationally use the scientific and practical potential of digital solutions in interdisciplinary interaction of specialists dealing with medical neurorehabilitation, significantly expanding the coverage of the population telerehabilitation services after a stroke.
    F i n d i n g s . Despite the positive experience of some medical organizations in Russia in the use of telerehabilitation, this direction is still in its infancy and is likely to develop in the coming years for the rehabilitation of many stroke patients, which will help healthcare organizers, doctors and patients and other interdisciplinary specialists effectively and interact seamlessly in the digital health ecosystem in the context of the development of patient-centered medicine.

  • 2022 № 2 Predicting the development of hypertension using machine learning models in the remote cardiomonitoring subsystem

    One of the tasks of personalized medicine is to build a new organizational model for providing medical care to patients, based on the selection of individual medical, diagnostic and preventive agents that are optimally suitable for the peculiarities of the body.
    Modern artificial intelligence methods allow you to solve problems of this type.
    Purpose. The aim of the study is to construct and apply predictive logistic regression models and decision tree using machine learning techniques to identify patients at high risk of hypertension without the need for invasive clinical procedures.
    Materials and methods. A formed data set consisting of 395 patient records of Voronezh City Clinical Clinic No. 1 is used. Each record contains patient parameters: patient sex; patient age; body mass index; waist circumference; hip circumference; tobacco smoking status; alcohol use status; systolic pressure; diastolic pressure. Machine learning methods are used to build prognostic models.
    Results. Two models for predicting the development of hypertension are constructed, characterized by high indicators of classification accuracy: a logistic regression model designed to calculate the patient’s individual risk (accuracy 96%), and a decision tree model designed to predict the patient’s possible disease with hypertension and explain the reasons why this disease can occur (accuracy 94%).
    Findings. The expediency of using machine learning methods in constructing prognostic models for assessing the state of patients is shown, the possibility of creating a recommendation block based on the obtained models in the remote cardiomonitoring subsystem is indicated.

  • 2022 № 2 Network facilities of internal communication of a medical organization.

    For the effective work of employees, it is necessary to competently establish internal communication. To date, dozens of companies provide various network means of internal communication, but not all of them are suitable for a medical organization.
    P u r p o s e of the study is to study the network means of internal communication in a medical organization.
    M a t e r i a l s a n d m e t h o d s . Network facilities of internal communication, their advantages and disadvantages were analyzed and systematized in the article. The definitions, types of communication networks and communication management are presented. The rules for the security of personal data when using network communication tools are described. A survey of employees was carried out to identify the main facilities of internal communication used, as well as satisfaction from them on the basis of the State Autonomous Healthcare Institution “City Polyclinic No. 21”. A SWOT based on the survey results analysis was carried out.
    R e s u l t s . 85 employees participated in the study. It showed that the main facilities of electronic internal communication used are the WhatsApp messenger and the electronic document management system. WhatsApp isn’t convenient for a significant part of employees to use, also there are too many working chats and messages in this messenger. Employees don’t often use the electronic document management system and there is a significant part of users who don’t like using it.
    F i n d i n g s . One of the most convenient facilities of internal network communication is a corporate social network, which, however, is difficult to implement and expensive in terms of costs. A more accessible means of internal communication is the Telegram messenger.

  • 2022 № 1 About the ability of the Uniform State Health Information System to solve management problems

    The unified state health information system (UHISZ) is becoming a state-significant information system (IS), which is capable of solving any problems of preserving and strengthening the health of citizens.
    Purpose of the study is to substantiate the problems of the construction and functioning of the Unified State Health Information System, the solution of which will multiply the capabilities of the Unified State Health Information System to solve the problems of preserving and strengthening the health of citizens and look for new non-traditional ways to improve their health.
    Results. I n the article, the authors distinguish three levels of problems to be solved. The first level of problems is problems that can be solved at the highest (legislative) level. The second level of problems is the problems that must be solved at the regional level. The third level of problems is the problems faced by ordinary users of the Uniform State Health Information System – doctors and other medical workers.
    The article initially uses the position – “Any information system is a tool for solving management problems.” The authors trace the stages of creating informatization in the Russian Federation from the complex program “Development and implementation of automated consultative systems for diagnosis, prognosis and choice of treatment tactics in emergency conditions”, which substantiated the development of automated systems for the period 1978–1990 until 2018, when Russia was the National Healthcare Project was launched, which includes the Federal project “Creation of a unified digital circuit in health care based on the unified state health information system (Unified State
    Health Information System) ”. The article discusses the problem of the privacy of data on the health of a resident of the region and the privacy of knowledge of the factors that determine both the health of a particular resident of the region and those around him. Preserving the complete privacy of a citizen’s “personal space” will lead to the fact that the Unified State Health Information System will have the most general data on the health of a resident of the region. In this case, it will only be possible to solve those problems that characterize the health of the region’s inhabitants, which were solved without any digitalization. The second level of problems is associated with the work of “Uniform State Health Information System” and support programs operating at the level of the regional health care system. The main
    regional problems associated with “EGISZ” are: problems of licensing of auxiliary programs; problems of accounting for the quantitative parameters of the development of the Uniform State Health Information System and auxiliary programs; problems of control over the work of auxiliary programs; problems of control over the filling of the Uniform State Health Information System; the problem of “user-friendliness of the interface” (especially for departments with cases of “peak work” – emergency departments).
    Separately, it should be noted the issues that are not problems of the functioning of the Uniform State Health Information System, but more characterize the new opportunities that arise in the presence of the Unified State Health Information System, which has a wide range of knowledge about human health. Uniform State Health Information System allows to solve in a different way the traditional tasks of an informational, organizational, medical, economic, scientific and educational nature that exist in the region. The noted fact prompts to look for new problems of assessing the health of residents of the region and ways to solve them, which were previously unavailable due to the lack of systematized data on the treatment and diagnostic process, other aspects of the activities of medical organizations, as well as information on factors affecting the health of residents of the region.
    Conclusions. Any information system is a tool for solving management problems. Without eliminating the problems of replenishing the Uniform State Health Information System with data on the factors influencing the health of the inhabitants of the region, it is impossible to effectively solve the problems of preserving and strengthening the health of the citizens of the region. The data, which, in accordance with the Federal Law “On Personal Data”, are included in the list of closed data, are encrypted in the EGIZS. To oblige the regional health authorities to introduce a licensing mechanism for programs interacting with the regional Unified State Health Information System.
    The development of regional Uniform State Health Information System will lead to a radical change in the view of the traditional tasks of information, organizational, medical, economic, scientific and educational nature, solved in the region.

  • 2021 № 10 THE MORBIDITY DYNAMICS MODELING AS AN INFORMATIONAL BASE TO MEDICAL CARE MANAGEMENT FOR THE OVER WORKING AGE POPULATION

    P u r p o s e o f t h e s t u d y : The substantiation of the need to use modeling of the dynamics of primary and general morbidity of the elderly and old population to improve the management of medical and preventive care for this age group.
    R e s e a r c h m e t h o d s : analytical, modeling and mathematical-statistical methods.
    R e s e a r c h b a s e . This study was conducted on the basis of the Moscow region with a population of 7,7 million people as of 01.01.2021.
    R e s u l t s . Shown, that use of mathematical modeling of the morbidity rate as an information database is one of the most optimal ways for calculating and assessing the needs in the volume of medical care for specific age groups of the population. The use of a nosological approach is proposed, because it allows to assess the required volume of medical care based on the existing pathology not on existing organizational models that may change. Morbidity modeling and forecasting is designed to promote optimization of medical and preventive examinations for elderly and old people in terms of determining the necessary forces and means based on knowledge of the magnitude of the existing need.
    C o n c l u s i o n . Taking into account the forecasting data, the use of the proposed models makes it possible to form an idea of the magnitude of the need and to monitor the implementation of state guarantees for the medical provision of elderly and old people.

  • 2021 № 8 Evolution of information systems

    In medical organizations, information systems solve the following tasks: the ability to receive and store information, quick access to it and its transmission, the ability to generate various reports, the availability of specialized workplaces of medical workers, etc. Information systems are gradually evolving, turning from a tool capable of solving the simplest management tasks into a tool capable of solving the entire range of management tasks encountered in the practice of a medical organization at all levels of management. The purpose of the study is to describe the four stages of creating information systems, highlight the characteristic features of the functioning of information systems at each stage, as well as highlight the list of the main management tasks that are being developed at the selected stage and ways to solve them. The main attention was paid to the development of information systems of medical organizations working with the children’s population.
    Results. The first stage of the development of information systems began with the use of computer technology in the practice of medical organizations and was reduced to the fact that programmers of both medical and third-party organizations began to develop tasks, the totality of which later became known as “creating an information system” of a medical organization. The goal of the second stage is already the transformation of information systems of medical organizations in order to describe production processes in terms of a process approach and implement them in the form of software modules. The third stage of building information systems is the stage of describing the work of a medical organization based on mathematical models in order to justify the optimal solution of the production tasks available in it. The fourth stage of building information systems is the construction of an “information system that implements the functions of an intelligent management system of a medical organization”. The authors of the article give recommendations on the formalization of information available in a medical organization for the purpose of its
    effective use by artificial intelligence.
    Findings. Thus, an information system is a tool for solving management problems, its development as a tool for solving management problems requires knowledge of management theory, organization theory, process approach in management, methods of multidimensional statistical analysis and modeling methods, languages and modeling methods based on neural networks.

    E-health is technologies that allow bringing medical services closer to patients in accordance with the objectives
    of state policy to level the staff shortage and remoteness of medical centers in rural areas. The established factors
    that reduce the quality of working life by the example of a survey of 248 midwifery specialists working in rural areas were characterized by the territorial proximity of a medical organization to home (70.1 ± 5.8%), identified the 5th rank place in the structure of measures to increase satisfaction with the quality of work, creation of a unified information environment. Analysis of modern literature has revealed the main aspects of the need and benefits of introducing e-health, such as accessibility, acceptability and quality.

  • 2020 № 9 Distributed ledger technology in patient experience management

    Actuality. The role of “patient experience” in assessing quality of medical services is growing as a global trend to value-based healthcare. Currently, only 11% of the Russian population positively assess the quality of medical services. Federal law
    No. 256-FZ requires all public healthcare providers to conduct an independent assessment of the provision of services, but 44% of
    organizations do not have such assessment, that can be caused by methodological and organizational reasons.
    Purpose of the study to evaluate the prospects for digital improvement of measuring patient satisfaction using distributed ledger
    technology. To analyze the literature on the topic of independent assessment of the quality of services provided by medical organizations; to analyze modern approaches to assessing patient satisfaction with healthcare providers; to assess the opportunities of improving the processes and results of satisfaction assessment using distributed ledger technology.
    Materials and methods. To achieve the purpose, the authors analyzed numerous relevant Russian and foreign sources of literature. Literature review was carried out across scientific libraries as eLIBRARY, PubMed by key words. The search in open sources in
    Google and Yandex was used.
    Results. The complexity of “patient experience” measurement, biased assessment, ambiguity of the results obtained and the difficulty of correct interpretation require more transparent methods of measuring the value of healthcare. A decentralized platform based on distributed ledge technology may become one of the possible solutions to the problem of objective assessment of patients ‘ satisfaction with the quality of medical services. Such approach allows to form an objective dynamic multi-factor rating of processes and all other components of healthcare services.
    Conclusion. The existing mechanisms for independent assessment of the quality of medical services need to be improved. Confidence in the results of the patient satisfaction measurement is one of core requirement. To make managerial decision evidence-based, it is important to evaluate the processes and patient experience along with results of healthcare services. Distributed ledger technology implemented as a dynamic multi-factor rating platform can be a logical solution for improving the quality of services and could become a part of a comprehensive IT ecosystem based on value-based approach.

  • 2020 № 7 Possibility of transboundary transfer of health-personal

    In the article, the authors analyze the possibility of cross-border transfer of personal data related to health, separately considering
    the possibility of such a transfer within the EAEU. The authors pay special attention to the analysis of the correlation of the concepts of medical confidentiality and personal data related to the state of health, the peculiarities of regulating the cross-border transfer of this information according to Russian legislation, as well as the consideration of the Unified State Information System in the field of health care created in the Russian Federation (hereinafter – the Unified State Health Insurance System, a unified system). The authors also consider the possibility of using a single system for cross-border data transfer, indicate the main risks of such use, and also determine the set of necessary additions for Russian legislation in order to use the EGISP in cross-border data transfer. Separately, the authors analyze the possibility of transferring personal data related to the state of health in the framework of the Eurasian Economic Union, which is often discussed within the EAEU digital agenda. The authors analyzed the permissibility of such transfer under the EAEU as a whole, the proposals put forward in its support, namely the adoption of a separate agreement about the delivery of health care by participating countries of the EAEU any citizens of member States of the Union, the adoption of the act on transfer of personal data about the health of the citizens of the EAEU member States and the creation of a single digital platform for the provision of such transfer (including, subject to the establishment
    of safety requirements the transfer of such data and penalties for violation of these requirements), as well as the identification of
    specially authorized state bodies in each of the EAEU countries in order to ensure responsibility for the transfer of this category of data.

  • 2020 № 6 Blood collection during the period of COVID-19 infection

    We searched Google Trends using terms in Russian and English: “coronavirus”, “blood transfusion”, “plasma transfusion”, “donate blood”. Object of study – requests submitted from February 15 to May 25, 2020 in the world, Russia and the USA. Anomalous increase in demand in the USA was revealed corresponding to the booming demand for plasma donors – convalescents of COVID-19: a) for “plasma transfusion” – March 30 – May 2, b) for “donate blood” March 17 – April 7. The studied Russian Google Trends indicators correspond to the reliable work of the blood service, which during the pandemic period solved the main task to provide hospitals with effective and safe components of donated blood. These data provide additional evidence of differences in the scientific validity of blood services in developed countries, and show that Google Trends can serve as a tool to track trends in blood donation.

    Nowadays one of the most important issue is the prediction of HIV and AIDS development. The ability to evaluate in numbers the outcome of the disease (in particular the AIDS development) is of great interest. It depends on a number of systematically taken medical indicators of an HIV infected person. For to create the information system for evaluating the severity of hiv infection in prisoners it is necessary, first of all, to evaluate the link between those indicators of the patient’s condition and the development of the disease itself. The article is focused on the possibility of using correlation analysis to identify the link between the develoopment of HIV and the data of the main medical indicators, which is regularly monitored in the penitentiary system. For to achieve this goal it was formed a sample based on the data of the Russian penitentiary system.

    This article presents a comparative analysis of two approaches for the telephone contacts management in a medical organization. The «traditional» model of the phone calls management in a medical institution and a model with the use of «call center» are considered.   • 2017 № 2 The performance management system of the healthcare institutions: experience of using information technology to enhance research productivity

    The original platform «Benchmarking research productivity» was developed in 2010 and integrated as a component of the research process management system in the NII KPSSZ. The designed platform addresses all the critical issues relevant for the management of the modern major health research institute. In the period from 2011 to 2015, the number of the articles published every year the research staff increased by 42.6%, the number of articles indexed in Web of Science and / or Scopus – increased 2.2 times; the number of citations for all publications increased 6 times; the average number of publications per 1 author increased by 27.1%. Information technology being one of central components allows to assess research productivity in the Research Institute management system and to monitor accurately the task performance by the local staff. The platform provides transparency of productive processes and gives opportunity to achieve main management objectives, i. e. motivation and coordination.
