  • 2023 № 5 Provision of the medical statistics service with qualified personnel.

    The implementation of the tasks of the medical statistics service is an integral part of the health care management process, as well as methodological support of the management vertical in the system [2, 3]. The quality of medical statistical and analytical data depends primarily on the personnel component of the medical statistics service. Currently, medical statistics are developing in the context of significant organizational changes in the healthcare system [1], which determines the priority of providing the service with qualified personnel [5].
    The purpose of the study: to study the availability of qualified personnel engaged in professional activities in medical statistics.
    Materials and methods. The data of the Federal statistical observation form No. 30 “Information about a medical organization” were used for the study.
    Statistical and analytical methods were used in the study.
    Results. The Medical Statistics Service performs one of the key roles in the management of a medical organization and the healthcare industry as a whole. In medical organizations, statistical document management is carried out by specialists of departments (offices) of medical statistics, which make up the medical statistics service as a whole. The key specialists of the service are statisticians and medical statisticians.
    The analysis of the indicator of the availability of qualified personnel of the medical statistics service shows its stable decline. Thus, in the dynamics of the total indicator of the provision of statisticians and medical statisticians per 10 000 population in the Russian Federation from 2015 to 2021, there is a decrease in its level by 18,6% or from 0,86 0/000 to 0,700/000, including in outpatient settings by 24,4% or from 0,450/000 to 0,34 0/000, in stationary by 11,8% or from 0,34 0/000 to 0,30 0/000. By the end of 2021, the shortage of qualified specialists of the medical statistics service amounted to 44,3% of their total needs. A similar situation is observed in medical organizations for outpatient and inpatient medical care (45% and 42,6%, respectively). Simultaneously with the decrease in the staffing of the medical statistics service, an increase in the load on the specialist was revealed. Thus, in medical organizations providing medical care in outpatient and inpatient settings, the average load on a statistician and medical statistician is almost 1,5–2 times higher than the average standard.
    Conclusions. The analysis of personnel changes in the medical statistics service indicates the need to improve approaches to the training and use of qualified personnel engaged in professional activities in medical statistics.

  • 2022 № 9 Intentional potential of scientific and clinical heads as a strategic resource of a healthcare organization

    R e l e v a n c e . The healthcare management system requires high-quality and rational use of the organization’s workforce, i. e. the skills of employees. The intentional potential of an employee is a set of needs, motives, etc., the «core» of the personality driving it to achieve a meaningful goal. The N. V. Sklifosovsky Research Institute for Emergency Medicine is a leading multidisciplinary medical institution with an established multi-level system and management culture, the quantitative assessment of the intentional potential of which is of great interest.
    P u r p o s e : to study the intentional potential of the heads of clinical and scientific departments.
    M e t h o d s . An anonymous survey was conducted among the heads of the scientific and clinical departments of the N. V. Sklifosovsky
    Research Institute for Emergency Medicine (2021), including two methods of S. Schwartz: assessment by the level of normative ideals (SVS) and by individual priorities (PVQ). The procedures of centering and ranking by the average values of each valuemotivational type were carried out. Next, the value-intentional consistency (VIC) and the coefficient of value oppositions (Cvo) were investigated.
    F i n d i n g s . No statistically significant differences in the VIC at the SVS and PVQ levels, as well as no differences in the Cvo at the SVS level in the groups of heads of clinical and scientific departments were revealed. Intrapersonal contradictions between the value oppositions «Security» and «Stimulation» at the PVQ level (p<0.05) were revealed, they are less pronounced in the group of heads of scientific departments. There were also no statistically significant differences in the other oppositions of value orientations of the PVQ level.
    C o n c l u s i o n s . The presented results can be used as additional factors in the planning and implementation of measures of sociopsychological management of scientific and clinical departments. In the professional selection for the relevant positions, the Cvo indicator is more informative. Differences between the groups of scientific and clinical heads in the «Security – Stimulation» Cvo were shown, the distribution of this indicator tends to be more pronounced among the group of scientific heads.

  • 2021 № 8 Regional features of the provision of medical personnel in Russia

    The article analyzes the regional features of the provision of medical personnel in Russia in the context of various subjects and federal districts.
    P u r p o s e of the study is to analyze the features of staffing of medical organizations in the constituent entities of the Russian Federation in modern conditions and the main directions of policy to improve the staffing situation at the regional level.
    M a t e r i a l s a n d m e t h o d s . According to the official statistics, an analysis of the staffing of medical and paramedical personnel was carried out in the context of the constituent entities of the Russian Federation; in the course of the study, methods were used such as: analytical, statistical, study and generalization of experience.
    R e s u l t s . The analysis showed that the provision of doctors and the provision of emergency medical services for different regions of the Russian Federation is different. The lowest values of the indicator of the provision of doctors in 2019 were recorded in the Ural FD, the Southern FD and the North Caucasian FD, while in the constituent entities of the Russian Federation the most pronounced tendency towards a decrease in the level of provision nursing staff was noted in Moscow, Tyumen region, Chukotka Autonomous Okrug, Republic of Kalmykia, Udmurt Republic, as well as in Yaroslavl, Murmansk, Novgorod and Kirov regions, which emphasizes the need to take appropriate management measures, both at the local and federal levels.
    F i n d i n g s . The existing imbalance in the country between the provision of the population with doctors and nurses in various regions (the difference between low and high rates is more than 2.3 times), as well as between the provision of medical workers in the city and rural areas is the main problem of staffing the system. To overcome this situation, further implementation of new measures of social support and moral incentives for medical personnel in Russia is required.

  • 2021 № 4 PROFESSIONAL BURNOUT OF NURSES: FEATURES OF GENERATIONS

    Professional burnout among nurses is an urgent public health problem, which negatively affects the quality of medical care. A specialist’s belonging to a certain generation determines its own characteristics of the response to stress factors and the development of professional burnout.
    Purpose of the study is to determine the main patterns of occurrence of professional burnout among nurses depending on the generation for development and implementation of targeted prevention methods.
    Materials and methods. A one-stage comparative study was planned and conducted using a specially designed questionnaire, including a burnout questionnaire MBI (Maslach Burnout Inventory) and questions assessing professional performance and socioeconomic status to determine the factors of professional burnout. All nurses were stratified into three generational groups.
    Results. The study involved 2486 nurses. It was found that respondents of all generations were prone to professional burnout, with a high and extremely high degree. At the same time, the third generation was most susceptible to the development of professional burnout, the first was the least. The main predictors of the development of professional burnout in the third generation were: lack of sleep and rest, low salary and large loan payments, excessive work load; in the first – the presence of chronic diseases. The second generation had a smaller percentage of respondents with an extremely high degree of professional burnout, because they were characterized by lower labor intensity, lack of sleep deficit and the highest salary.
    Findings. Thus, prevention measures can be individual for each generation, with considering the differences in predictive and protective factors. While, it is necessary to focus on the youngest, third generation, reinforcing factors preventing the development of professional burnout.

    The problem of staffing in healthcare remains relevant for a long time. The article considers the possibilities of participation of local self-government bodies in attracting and securing medical workers, primarily primary care. The
    analysis of normative legal acts of the Federal level is presented. Amendments to Federal laws № 131-FZ and № 323-FZ related to specifying the powers of local self-government bodies in terms of health personnel are proposed. The necessity of a comprehensive assessment of the implemented measures of social support for medical workers to identify the most effective measures and select the best practices is justified.

  • 2021 № 1 Emotional burnout of a district doctor

    Based on the results of assessments of the dependences of the level of psychoemotional load of a primary care
    physician (PHC) (district physician) on medical and social problems related to the conditions of his activity, measures have been developed for stress resistance and health saving of a district physician (VTU). The Orenburg region with a population of almost 2 million people was the pilot territory (base) for the study. The region has 12 cities and 35 rural districts, the share of the rural population is 40.1%.
    Research materials und methods: sociological, direct observation, statistical: 1014 questionnaires of survey of district
    physicians (93% of the total number of VTU) were subjected to statistical processing. The study included 4 stages. At the first stage, sociological surveys of district physicians working in medical organizations located in urban and rural settlements were carried out (568 and 446 VTU, respectively). At the second stage of the study, the level of psychoemotional burnout was assessed using the modified method of V. V. Boyko (2010) “Diagnostics of the level of emotional burnout”. At the third stage of the study, we calculated the statistically significant correlation of the symptom of professional burnout (SPEV) with factors related to medical and social problems of the professional activity of the local GP. At the fourth stage of the study, a set of measures has been developed and is being implemented to increase emotional stability in the professional activity of the local GP.
    Relevance. WHO classifies occupational stress as a disease of the twentieth century, which can manifest itself in any
    profession and can reach the size of a “global epidemic”. The accumulation of negative emotions in the profession of a
    doctor can lead to psycho-emotional exhaustion and the formation of the syndrome of “emotional burnout”. A necessary quality of a primary care physician is emotional stability. This is the ability to overcome a state of excessive emotional arousal. Of particular importance is the development of professional burnout and the assessment of professional stability for district physicians who make first contact with the patient.
    Results. Measures have been developed to increase the effectiveness of emotional (professional) stability in the activities of VTU.
    Conclusion: Emotional stability in the doctor’s work allows you to maintain and further increase the effectiveness of professional activities. The obtained research results were used for the development of the “program for the modernization of health care in the Orenburg region”. They were used in the development of the Software product of the Ministry of health of the Orenburg region.

    The strategy for the development of healthcare in the Russian Federation improves the staffing of the healthcare system, identified as a priority. The state programs set target indicators for the provision of the population with medical personnel, measures to equip medical organizations with medical workers, and measures of social support for medical workers. The aim of the work was to study the factors relevant for graduates that determine the choice of place of work and the development, based on the data obtained, of measures aimed at improving the availability of qualified medical personnel for medical organizations.
    The results of a sociological study of graduates of a medical university on the problems of employment in medical organizations are studied.
    Analyzed: indicators of the population of the region with doctors; the need for medical organizations of the region in medical personnel; events implemented in the region in order to provide medical organizations with qualified personnel.
    The measures, the implementation of which is aimed at increasing the availability of medical organizations with qualified personnel and achieving the effectiveness of employment of graduates of the university, are determined.

  • 2020 № 5 Availability and staffing of gastroenterologists in the Russian Federation and in pilot regions, 2013–2017

    Human resource issues are of high relevance for accessibility of the quality of health care. The aim of the study was to conduct analyses of actual staffing of health care facilities for provision of gastroenterological care on an outpatient basis in the Russian Federation and its subjects. Materials and methods: calculation of occupied positions and physicians-gastroenterologists rate, extensive indicators, method of descriptive statistics. Results. Multidirectional levels and trends in physicians-gastroenterologists coverage rates in the constituent entities of the Russian Federation have been established. In a number of federal districts there is an increase in the studied indicators, in some – levelling. Conclusions. An analysis of the availability of gastroenterologists roviding health care an outpatient basis showed that the supply of gastroenterologists in the federal districts increased or levelled. Moreover, in six of them there was a decrease in the staffing rate by these specialists. The results of the study could be used for the human resources planning and for decision-making.

  • 2020 № 4 Training in clinical recommendations and assessment of the level of knowledge of doctors involved in providing primary health care, using remote educational technologies

    The implementation of clinical recommendations is an urgent task, and it is important to maintain continuity between primary health care and specialized medical care. Distance learning courses were developed for 25 modules, and a testing system was created, which was implemented during the training of 1928 doctors from 128 medical organizations in three regions. The purpose of the work was to analyze and evaluate the effectiveness of the developed and implemented set of measures for training and testing doctors in three regions of Russia who participate in primary health care, on the implemen- tation of clinical recommendations.
    Data analysis was performed using the IBM SPSS Statistics 16 software package. The samples were checked for the normality of the distribution using the Kolmogorov-Smirnov method. A nonparametric Wilcoxon test for dependent samples was used to assess the statistical significance of differences in cases where the distribution in the samples differs from the normal one. The level of statistical significance was defined as p ≤ 0.05.
    The program of work consisted of five stages and included the development and implementation of distance courses in the format of multimedia lectures for doctors providing primary health care, studying a number of clinical recommendations, as well as conducting primary and repeated testing with automated evaluation of results for groups of clinical recommendations at the level of the region, medical organization and specific doctor.
    The original software made it possible to evaluate the initial and received level of knowledge, to get differentiated results online, and on this basis to give reasonable targeted recommendations for further training of a particular employee, as well as for planning training events in the region as a whole.
    The developed methodology of distance learning cycles allows to provide training of a significant number of doctors in a rel- atively short time with operational control of the quality of knowledge of doctors of various specialties, which is why the tested system can be easily scaled and adapted to solve any problems to improve professional competence of doctors.

  • 2019 № 10 Professional standards as a tool to improve the efficiency of personnel management in medical institutions

    The transformation of the sphere of employment and the labor market of hired personnel, training systems for specialists have identified the need for the development and implementation of professional standards. The article substantiates the main reasons for the demand for new regulatory documents to the challenges of the time, the relevance of the competency-based approach at all stages of the implementation of professional standards in the healthcare system. The implementation of professional standards requires large-scale processing by the personnel department of organizational and structural documents; adaptation to new per-sonnel management tools, while taking into account the interests of employers, workers, educational organizations engaged in the training of specialists; additional costs for retraining workers. The main problems of the implementation of professional standards in the healthcare system are related to the renaming of posts and the transfer of employees to less qualified posts, which affects the level of wages and benefits, provokes conflict situations in the team, and creates the conditions for professional burnout.
    The article shows the practice of implementing professional standards, implementation problems and ways to solve them using the example of a medical institution, considers changes in the training system, reflecting the dynamics of the formation of professional competencies in the face of uncertain changes. Particular attention is paid to the problems associated with the implementation of professional standards in healthcare.

  • 2019 № 9 The problem of personnel shortage of the doctors for medical prevention

    The Federal Law of November 21, 2011 № 323-FL «About the basis of the protection of public health in the Russian Federation» defined the priority of prevention in the field of health protection (Article 12). However, a number of problems in staffing of the system of medical prevention remain. Aim: to investigate the dynamics of the number of posts for doctors for medical prevention (positions, occupied positions, individuals), as well as their staffing level and combination rate for the period from 2014 until 2017 in the Russian Federation and its regions. Methods: based on the data of the Federal Statistic (form № 30), the article presents a statistic analysis of the number of posts for doctors for medical prevention for the period from 2014 until 2017 in the Russian Federation and its regions. Results: There is a low staffing level for doctors for medical prevention in the Russian Federation, which decreased even more over the study period: from 73.2% to 64.0% (by 12.5%). This is explained both by an increase in the staff positions of these doctors in the country from 1031 to 1526 (by 48.0%), while the number of occupied posts increased only by 29.50% (from 754.25 to 976.75), and by a simultaneous decrease in the part-time combination rate from 2.1 to 1.7 (19.0%), which continues to be high. At the same time, the num¬ber of regions with 100.0% staffing level of these personnel decreased by 76.9% (from 13 to 3). Conclusion: The low staffing level (64.0%) and the high part-time combination rate of doctors for medical prevention (1.7) in the Russian Federation in 2017 indicate a shortage of individuals for doctors for medical prevention – the main workers in the occupied positions and their replacement by doctors – co-workers of other specialties. This largely takes place due to the absence of the specialty «medical prevention», which necessity of the introduction needs to be discussed by the medical community.

  • 2019 № 6 Medical staff resources of the Moscow psychiatric service during its reform

    Medical staff resources of the Moscow psychiatric service during its reform. Since 2010, a modernization of the psychiatric service has been carried out in Moscow. Reforms affect both outpatient and inpatient units. An analysis of the staff resources of the service reflects the effectiveness of the reforms. The aim of the study was to improve the quality of psychiatric care based on the analysis of staff resources of the Moscow psychiatric service. The article presents data on the staff resources of the psychiatric service in 2008-2017. During the modernization of the psychiatric service, the number of psychiatrists was at a high level, the provision of psychiatrists to the Moscow population of was higher than the average provision to the Russian Federation. Growth in the number and staffing of ambulatory rates in psychiatric institutions in Moscow in 2015-2017 partially compensated for the decrease in the number and staffing rates in psychiatric hospitals, however, there is currently a staff shortage in the psychiatric service of Moscow, as indicated by the excess of job-sharing coefficients in individual institutions. During the study period, there was a positive change in the number of visits per 1 psychiatrist.

  • 2019 № 2 Health resourcing of traumatologic-orthopedic service in the subject of the Russian Federation amidst realization of grant program “Zemsky doctor

    Medical staff are one of the crucial factors, which guarantee health care service efficiency. However, Russian national traumatologic-orthopedic service exists under the lack of significant lack of staff. Grant program “Zemsky doctor” became one of the problem solving ways.
    Purpose. To evaluate Health resourcing of traumatologic-orthopedic service, and to examine results in completing staff jobs of trauma orthopaedists in medical organization in the subject of the Russian Federation achieved due to “Zemsky doctor” program realization.
    Approaches: analytical, statistical.
    Results. The research is conducted within Stavropol region. It showed that in 5 municipal districts of 36 trauma orthopedist jobs were either not specificated or completed, 6 of 36 had no traumatologic beds. Within the period 2011–2016 we registered negative dynamics of trauma orthopedists completeness from 94,4% to 88,9%. Comparing to 2011, when excluded trauma orthopedists were 23,3% of the general number of fired and hired, in 2016 they were 42,8%. The most active medical staff change is in town medical organizations. Within the period of 2012 to 2016 in terms of “Zemsky doctor” program 12 trauma orthopedists were hired in Stavropol region, which is 1,9% of all signed contracts under this program and 9,2% of all hired within this time trauma orthopedists.
    Conclusion. We revealed significant disproportion of health resourcing in health service centres in the subject of the Russian Federation, which limits traumatologic-orthopedic care accessibility.   • 2018 № 9 The number of plastic surgeons in the Russian Federation and their activities in 2017 year

    Relevance. The issues of staffing of medical organizations always remain one of the urgent problems of practical public health. In the period of wide development of such a healthcare sector as plastic surgery, the urgency of providing specialist doctors and organizing control over their activities is growing. Purpose of the study. Conduct an analysis of the security and activities of plastic surgeons in the context of the subjects of the Russian Federation. Material and methods. The data of the federal statistical Observation № 30 “Information on the medical organization” for 2017 are used. Statistical, analytical, descriptive methods of statistics were used in the work. Results. The majority of plastic surgeons work in the medical organizations of the Central (36,2%), Siberian (20,7%) Privolzhsky (15.5%) federal districts, mostly in hospitals. At the same time, the largest number of specialist visits is in the Southern (49,7% or 8,330 visits per year) and Siberian (37,8% or 6335 visits per year) Federal Districts. Conclusions. An analysis of the number of plastic surgeons showed that most of them work in the occupied positions in the Central Federal District, especially in Moscow. At the same time, the highest number of specialist visits is in the Southern Federal District.

  • 2018 № 1 Availability and staffing by phthisiatricians in the Udmurt republic, 2011-2015

    Provision of human resources represents one of the main problems in the organization of health care. The purpose of the study was to determine the main trends in the provision and staffing of phthisiatricians. The purpose of the study is determination of the main trends in the provision and staffing by phthisiatricians. Material and meth¬ods: official statistics of Udmurt Republic from 2006 to 2015. Mathematical, analytical and descriptive methods were used. Analysis of the availability of outpatient clinics by the phthisiatricians of the Udmurt Republic in the dynamics for 2011-2015 (per 10 000 population) showed the stability of the situation in the provision of specialists with a slight decrease in staffing. A positive trend was noted in the desire of young specialists to work in phthisiatric services. The results of the study of the level of provision and staffing of the phthisiatrics services in Udmurt Republic demonstrate the stability of the situation as a whole across the Republic, although in some administrative regions of the country the data are highly unequal. The arrival of young specialists in the phthisiatric service is encouraging growth in the supply of specialists in the near future

    In accordance with paragraph 3 of the Rules for the development and approval of standard labor standards approved by the Government of the Russian Federation on November 11, 2002 No. 804 (Collected Legislation of the Russian Federation, 2002, No. 46, Article 4583) and paragraph 19 of the action plan (“road Card“)” Changes in social sectors aimed at improving the effectiveness of healthcare “approved by the Decree of the Government of the Russian Federation No. 2599-r of December 28, 2012 (Collected Legislation of the Russian Federation, 2013, No. 2, Article 130, No. 45, Articles 5863, 2014, No. 19, Item 2468), in 2016 photo-timing observations of the working process of phthisiatricians, doctors-surgeons and doctors-dermatovenerologists were conducted. The work was conducted in accordance with the methodology of conducting photo-timing studies within ten working days for two physicians in each specialty. The results of the research showed the need to develop new normative documents on setting the time limits for visiting one-patient TB doctors, surgeons and dermatovenereologists who provide primary care in outpatient settings (not providing 24-hour medical supervision and treatment).

    The demand for increasing efficiency in the use of health resources, including human resources, require improved approaches to demand determination for non-medical personnel of health. Over the past three years, almost half (47.3%) increased the absolute number of «other staff» and medical organizations increased by 24.6%, its share in the total number of health workers. Analyzed data provided by the health authorities of the Russian Federation on the size, composition and the additional requirements of healthcare organizations-medical and other staff. The necessity of developing a unified nomenclature of positions in the non-medical part of the health workforce, clearly defined methodological approaches to the formation of the staffing structure and numbers of non-medical personnel depending on the type and capacity of the medical organization.

    The study of the health of students of medical colleges run by the complex medical and demographic and socio-sanitary program, which included an assessment of the influence on it together regional demographic, environmental, social and economic prerequisites for registered biomedical characteristics of the organism. The study was developed and tested a complex methodology that takes into account medical and demographic and socio-hygienic characteristics as bases for the study of health, structure and disease, and to identify the health status depending on the conditions and the way of life of students of medical colleges. The results of the study, which involved more than 180 000 students from 347 medical colleges and technical schools of the Russian Federation. Representativeness of the sample was provided with a statistical error of 5%. Minimum sample size for the study of the views of college students estimated adjusted for finite population is 120 thousand observations.
