  • 2023 № 9 Self-assessment of reproductive health by future doctors.

    The reproductive health of the younger generation is of particular medical and social importance. The state of health of today’s youth, their reproductive potential and attitudes will be reflected in the demographic situation of Russia’s future. To maintain and improve the reproductive health of young people, a whole range of medical measures is being implemented. Of interest is the state of reproductive health of future doctors who receive a comprehensive education in the field of health care and have the highest level of knowledge about risk factors and health-saving technologies.
    T h e p u r p o s e o f t h e s t u d y : to analyze the responses of medical students to assess their reproductive health and commitment to maintaining it.
    M a t e r i a l s a n d m e t h o d s . An anonymous survey of 460 medical students studying at medical universities in St. Petersburg was conducted (September-October 2022). Methods of analytical and descriptive statistics were used.
    R e s u l t s . There is a low effectiveness of activities to inform adolescents about sexual education and reproductive health. Most students do not consider their own health as a priority for the coming years, give a low assessment of their physical condition, rarely go in for sports or do not do it at all, every second eats irregularly. A third of young people suffer from acute and chronic extragenital diseases, and every fifth has a pathology of the reproductive system. 72.6% of girls and only 21.4% of boys visit a gynecologist/urologist/andrologist at least once a year, more often for preventive purposes. During sexual intercourse, respondents prefer barrier contraceptives that meet the requirements of availability, reliability and ease of use.
    C o n c l u s i o n s . When developing strategies for improving and maintaining reproductive health, gender differences, medical and social characteristics and motivation of young people should be taken into account. To improve reproductive health-saving behavior, it is necessary to increase the availability of preventive measures, incl. anonymous counseling.

    The article presents the results of the analysis of the practice of using working time by the heads of nursing services of outpatient medical organizations providing primary health care. The data obtained will help the chief and senior nurses of health organizations to optimize professional activities in the management of nursing personnel by improving the organization of work processes and increasing the effectiveness of nursing work. Given the fact that organizational problems that impede the rationalization of managerial work, identified during the study, are mostly universal, the proposed recommendations can be used in the management of managers – health managers of any level and profile of medical activity working in state and non-state medical organizations any legal form.

  • 2023 № 12 On the Formation of Innovative lean Competence of nursing staff providing assistance to the population in the PSMP System.

    Introduction. The article proposes and proves the effectiveness of a model for the formation of innovative lean competencies among nursing staff operating in an optimized healthcare environment.
    Goal: to determine the effectiveness of the implementation within the framework of postgraduate training of an educational module on the formation of innovative lean competence in the professional activities of practicing nursing specialists.
    Methods. The study was conducted according to a specially developed questionnaire in two stages: the first – in 2019 (before the implementation of the educational module); the second – in 2022 (evaluation of the implementation results). The analysis of the results provided for the calculation of relative values (P) and their errors (± m). The same indicators were compared in dynamics, the Student’s coefficient (t) was used to assess the reliability of the differences. The differences were considered statistically significant at p<0,05. Microsoft Office Excel and Statistica 5,0 programs were used to create an electronic database and calculate indicators.
    Results. At the first stage, a high readiness of the majority of nurses for innovation was revealed, but a number of limitations for innovation were recorded: low level of computer skills, lack of necessary competencies for implementing innovations, lack of leadership qualities, search for personal gain. A strong inverse correlation was found between age and the desire to introduce innovations. The results of the second stage showed a 1,4-fold increase in the share of nursing staff who want to introduce innovations in their workplace; a 1,8-fold decrease in the proportion of people who are wary of changes in their professional activities; reducing the time spent on processing
    medical documentation and increasing the amount of time allocated to working with patients. The proportion of nursing specialists using an electronic document management system in their professional activities has also increased – from 1,6% to 41,6%.
    Conclusion. The effectiveness of the proposed educational module for the formation of innovative lean competence in the professional activities of nurses providing assistance to the population in the primary health care system is confirmed by the achieved positive results of its testing in healthcare institutions.

  • 2021 № 4 PROFESSIONAL BURNOUT OF NURSES: FEATURES OF GENERATIONS

    Professional burnout among nurses is an urgent public health problem, which negatively affects the quality of medical care. A specialist’s belonging to a certain generation determines its own characteristics of the response to stress factors and the development of professional burnout.
    Purpose of the study is to determine the main patterns of occurrence of professional burnout among nurses depending on the generation for development and implementation of targeted prevention methods.
    Materials and methods. A one-stage comparative study was planned and conducted using a specially designed questionnaire, including a burnout questionnaire MBI (Maslach Burnout Inventory) and questions assessing professional performance and socioeconomic status to determine the factors of professional burnout. All nurses were stratified into three generational groups.
    Results. The study involved 2486 nurses. It was found that respondents of all generations were prone to professional burnout, with a high and extremely high degree. At the same time, the third generation was most susceptible to the development of professional burnout, the first was the least. The main predictors of the development of professional burnout in the third generation were: lack of sleep and rest, low salary and large loan payments, excessive work load; in the first – the presence of chronic diseases. The second generation had a smaller percentage of respondents with an extremely high degree of professional burnout, because they were characterized by lower labor intensity, lack of sleep deficit and the highest salary.
    Findings. Thus, prevention measures can be individual for each generation, with considering the differences in predictive and protective factors. While, it is necessary to focus on the youngest, third generation, reinforcing factors preventing the development of professional burnout.

  • 2023 № 11 Factors affecting the shortage of nursing staff in medical institutions.

    The article presents the literature data on the shortage of nursing staff in medical institutions. The characteristic of the base on which the research was conducted is given. The analysis of completing of the nursing staff of the SAHI «Center for the Protection of Motherhood and Childhood» was carried out. It was found that during the three years of the Center’s existence, there was a shortage of secondary medical personnel from 50.0% to 65.0%. The study identified factors affecting the formation of a personnel shortage. The main ones were: dissatisfaction with the management style of the Center (57.2%), a high risk of overstrain and stress when performing functional duties, dissatisfaction with wages that do not correspond to professional loads. The insufficient involvement of young personnel under the age of 30 and the lack of motivation to obtain qualification categories when working in this medical institution had a negative impact on the replenishment and preservation of personnel potential. Based on the results of this study, ways to solve the problem of shortage of nursing staff in the Center have been developed.

    Innovative technologies use in the daily nurse practice makes their work more professional, comfortable, ensures safety and convenience of performing basic professional duties, reduces labor costs, and allows to quickly and efficiently ensure the implementation of new diagnostic and treatment process.
    Aim: analysis of the experience in the development and implementation of SOPs for nurses for 2018–2019. in Central
    City Clinical Hospital № 6, Yekaterinburg.
    Methods. The work analyzed reporting documents (SOP implementation logs for each subdivision of Central City Clinical Hospital № 6, Yekaterinburg during 2018–2019 period), characterizing the number, type of developed SOPs, depending on their purpose, the timing of their implementation.
    Results. During 2018–2019 period there have been all main nomenclatures SOPs developed and implemented into the
    nursing practice. The main stages of the development of SOPs are analyzed, illustrative examples are given. Encountered by the staff of the medical institution SOPs development and implementation related difficulties are discussed. Main prospective activity directions in the field are determined.
    Conclusion. On the basis of municipal Central City Clinical Hospital № 6, Yekaterinburg, SOPs development and implementation for various types of nursing activities have been successfully carried out.
