  • 2021 № 10 REVIEW. INTERNATIONAL ACCREDITATION IN COMMONWEALTH OF INDEPENDENT STATES CLINICS AND WORLDWIDE PRACTICE

    Accreditation is a process of external evaluation of clinics, and is accepted worldwide as a tool for healthcare quality improvement.
    There is a number of international accreditation commissions, who carry out accreditation for medical organizations. The most widely recognized is Joint Commission International. There is also a head accreditation commission, called ISQua. They accredit commissions, standards and educational courses. Among CIS countries, Kazakhstan owns the leadership in the number of JCIaccredited clinics. Analysis of international experience showed that accreditation could have its positive effect on organization of everyday work of clinic, treatment outcomes, patient satisfaction and economic efficiency. Accreditation is a helpful tool to make optimal the general level of the medical service performed by organization.

  • 2020 № 10 Telemedicine in the EAEU countries: reality and prospects

    This article is devoted to a comprehensive analysis of telemedicine in the countries of the Eurasian Economic Union:
    its legal regulation, methods of implementation and development prospects. The authors pay attention not only to a comparative legal analysis of telemedicine regulation in the EAEU countries, but also to determining the possibility of creating a general agreement on telemedicine between the Russian Federation, the Republic of Belarus, the Republic of Armenia, the Republic of Kazakhstan and the Kyrgyz Republic. Identifying gaps in the national regulation of telemedicine in these countries, the authors point out the aspects in which it is necessary to harmonize the regulatory framework for telemedicine consulting, and also provide for those provisions that must be necessarily disclosed in the agreement on telemedicine within the EAEU.

  • 2020 № 9 Forecasting of labor resources in the system of primary health care in Kazakhstan

    Primary health care is a highly effective and efficient way to tackle the root causes and risks of poor health
    status, and to address emerging problems that threaten future health and well-being.
    Goal: Develop a model for predicting the need for general practitioners (GPs) until 2030, taking into account various
    input data in the equations describing the movement of labor resources in the form of “flows” and “ stocks “ using the
    method of system dynamics.
    Methods: The model was built using the Any Logic program, based on information about the availability of primary
    health care doctors, demographic data and the general prevalence of diseases in the population. Three scenarios were
    considered to predict the need for general practitioners. The base year was 2018, the forecast was carried out until 2030.
    Results: All of three scenarios indicate that with the current number of graduated GPs, the shortage of primary care
    physicians will be exacerbated. In general, the shortage can reach more than two thousand.
    Conclusion: Government and medical universities need to take measures to correct the number of students in the specialty of “General Practitioner” in order to avoid a growing shortage of primary care physicians in the future until 2030.

  • 2019 № 10 Overview of interdepartmental programs for public health care

    “In the Decree of the President of the Russian Federation of 07.05.2018 No. 204 “About the national purposes and strategic problems of development of the Russian Federation until 2024” achievement of the following target indica¬tors is established: increase by 2024 in the expected life expectancy at the birth till 78 flyings and till 80 flyings by 2030)”. Prevention of noninfectious diseases on an interdepartmental basis is necessary for achievement of such indicators. We carried out the overview of federal orders, resolutions, a regulatory framework and interdepartmental programs for public health care. The analysis of literature showed that in the Russian Federation development and realization of the strategy, projects and programs directed to prevention of noninfectious diseases and forming of commitment of the population to a healthy lifestyle is conducted many of which are between sectoral and use the principles of “Health in all strategy”.

  • 2017 № 5 Study of workload and working time costs of general practitioners in healthcare organizations in Kazakhstan

    The article presents the results of a chronometric analysis of patients from the Karaganda policlinic, while analyzing and studying the costs of the doctor’s working hours for performing various labor operations. In addition, the main factors influencing the duration of admission of general practitioners of primary care organizations were identified, and we also compiled a forecast of the duration of GPs’ admission, depending on the treatment of some variables. Statistical analysis determined the main factors affecting the duration of admission of patients by GPs. These include the Communicative Factor or Component 1, Operating Procedures or Component 2, and Working with Documentation or Component 3

  • 2017 № 3 The possibility of adapting European clinical and economic models for use in the Russian routine practice to calculate the treatment of patients with diabetes mellitus type 2

    The cost of the type 2 diabetes mellitus (DM2) treatment with dapagliflozin as a first-line drug, its subsequent combination with metformin and then switching to insulin therapy, were 297 321 068 rubles. This calculation based on the prospective modelling with a 25 year prognosis horizon in the cohort of 1,000 patients. In case of the first-line saxagliptin application, its following combination with metformin and then switching to insulin therapy, the costs were 297 733 828 rubles, that is at 412,760 rubles more than first-line dapagliflozin strategy. A cost-effectiveness analysis of the first-line dapagliflozin strategy in comparing with the saxagliptin treatment is not highly sensitive to changes in the price that are within 10%. In this range of price changes, the dapagliflozin strategy can be considered even more cost-effective achieving 141.38 rubles for QALY gained with an increase in the price of dapagliflozin by 10% and 244.05 rubles / QALY, while the price of saxagliptin decreases by 10%.
