  • 2022 № 11 Development of patient-centered services in medical organizations

    The development of patient-oriented means of interaction between citizens and organizations of the participants of the modern “digital interaction” system is a current trend in the field of medical information technology. The formed principles of digital transformation of healthcare, which are carried out within the framework of the federal project “Creation of a unified digital contour in healthcare based on the unified state health information system (UGISZ) ”. National Project “Health Care”, provide ample opportunities for designing and developing new applied solutions in terms of requests from medical organizations for modern domestic IT tools for “communication” with patients.
    The aim of the article is to analyze and describe the current level of development of patient-oriented software products and methods of their development.
    As a result, the software “Personal account of the patient “Interin LC”, developed on the basis of research and running on a stack of freely distributed software, is described.

  • 2022 № 7 Data-based management in imaging: evaluation of the performance of a unified radiological information service model

    The development and availability of information technologies covers all spheres of society, providing ever newer and more advanced means of automating processes and structuring data, primarily in order to improve management efficiency.
    Radiation diagnostics, due to its clinical significance, is one of the key areas of modern healthcare. At the same time, it is distinguished by an extremely high degree of digitalization, which creates extensive opportunities for the development and improvement of organizational and managerial aspects. An analytical study was carried out on the principles of a systematic approach, consisting of three stages.
    First stage. The main phases and indicators of the development of a single digital circuit for radiodiagnosis in Moscow are characterized: the dynamics of the integration of diagnostic equipment, the structure of medical organizations, the rate of accumulation of the results of radiological examinations by modality.
    Second phase. The analysis of the influence of a single digital circuit of radiodiagnosis on the level of informatization of medical organizations and informatization of the health care system of a constituent entity of the Russian Federation was carried out, its compliance with the requirements of the Ministry of Health of Russia was assessed.
    Third stage. The analysis of the influence of a single digital circuit of radiodiagnosis on the level of digital maturity of the health care system of a constituent entity of the Russian Federation was carried out.
    The practical implementation of the model of a single digital circuit for radiation diagnostics of an administrative-territorial unit was carried out in the period 2015–2020 in Moscow.
    The Unified Radiological Information Service (UIS) of Moscow was created – an information system in the field of healthcare that combines the workplaces of radiologists, radiologists and diagnostic equipment, accumulates information about each study or a series of studies conducted on devices connected to it, is equipped with additional tools for solving analytical, managerial and organizational tasks.
    The analysis of the significance of ERIS EMIAS as an implemented single digital circuit for radiodiagnosis of a subject of the Russian Federation was carried out.
    The development of URIS EMIAS ensured a steady increase in the level of informatization coverage of medical organizations that have radiation diagnostics units in their composition.

    The situation in radiology (intensive growth of infrastructure against the background of organizational, methodical
    and managerial problems) requires a new system approach to organization and management. The concept of radiology
    reference center as a structure in the health care system is supposed to provide remote assessment, interpretation, description and control of radiological studies performed in medical centers of all forms of ownership, with the use of telemedicine technologies. The reference center should simultaneously solve organizational, methodological, therapeutic, diagnostic, educational problems, information and coordination tasks and scientific issues. The concept is based on systematized results of publications and own successful experience of creation and effective functioning of the reference center on the basis of the State Funded Healthcare Institution of Moscow “Research and Practical Clinical Center of Diagnostics and Telemedicine Technologies, Department of Health Care of Moscow”. Basing on the concept, it is necessary to keep developing the scientific and practical aspects of normative, legal and methodological support of the activities of radiology reference centers.

  • 2023 № 4 Evaluation of the effectiveness of using remote monitoring of blood pressure in the dispensary observation of patients with arterial hypertension

    Arterial hypertension (AH) is by far the most important risk factor for cardiovascular disease. An effective method of dealing with high blood pressure (BP) is dispensary observation. Of particular relevance is the search for new tools to improve the dispensary observation of patients with hypertension, one of which may be remote monitoring of blood pressure, as an option for providing medical care using telemedicine technologies.
    Purpose of study – Analysis of the effectiveness of using remote monitoring of blood pressure in the dispensary observation of patients with hypertension.
    Methods. A non-randomised, open-label, generalized, single-centre, retrospective, longitudinal, parallel, applied study included 78 patients who completed a remote blood pressure monitoring program who were subsequently divided into 2 groups depending on the degree of achievement of target values of blood pressure (<135/85 mm Hg).
    Results. During the observation period, all patients (n=78) experienced a decrease in SBP by 8.2 mmHg, DBP at 3.8 mmHg. Group 1 consisted of 35 patients who reached target levels of blood pressure, group 2 included 43 patients who did not reach target levels of blood pressure. Initially, the groups did not differ in sex and age, while in the second group, the initial values of SBP and DBP were higher. The study found that the degree of achievement of target levels of blood pressure is influenced by the following factors: the level of SBP and DBP at the beginning of observation, the proportion of extraordinary measurements with blood pressure levels above 135/85 mmHg during the observation period, the proportion of missed blood pressure measurements during the observation period, as well as the duration of remote monitoring.
    Conclusions. Remote monitoring of blood pressure is an effective tool for improving the dispensary observation of patients with arterial hypertension and can improve the availability and quality of medical care.

  • 2023 № 4 Determinants of the use of telemedicine in pediatric dentistry.

    Currently, favorable conditions are being created in the Russian Federation for the development and effective use of telemedicine in healthcare. Options for the use of information and communication technologies in pediatric dentistry are: telemedicine consultations, consultations, distance education of medical workers, medical education and preventive activities among the child population.
    The aim of the research as part of the dissertation work, the research was aimed at studying the attitude of dentists to the use of telemedicine technologies in pediatric dentistry.
    Materials and methods. The process of studying the attitude and readiness of pediatric dentists to use telemedicine technologies was aimed at identifying individual (micro) and conceptual (meso- and macro) factors affecting the use of medicine in pediatric dentistry. The study included a semi-formal interview and questionnaire.
    Results. As a result of the research on the sections: Telemonitoring – remote monitoring of patients, including outpatient management of patients; storage and transmission of medical data; interactive telemedicine, it was found that doctors in Tver and the Tver region have both positive and negative beliefs and opinions that can promote or hinder their use of telemedicine in their professional practice.
    Conclusions. Research with the participation of pediatric dentists will help both developers and managers of medical institutions to understand the limitations and advantages of technologies from the point of view of practicing pediatric dentists, will allow predicting its development.
    The results of this study will contribute to solving the problems of implementing telemedicine services, contribute to the rational development of telemedicine platforms in pediatric dentistry in Tver and the Tver region.

  • 2020 № 10 Telemedicine in the EAEU countries: reality and prospects

    This article is devoted to a comprehensive analysis of telemedicine in the countries of the Eurasian Economic Union:
    its legal regulation, methods of implementation and development prospects. The authors pay attention not only to a comparative legal analysis of telemedicine regulation in the EAEU countries, but also to determining the possibility of creating a general agreement on telemedicine between the Russian Federation, the Republic of Belarus, the Republic of Armenia, the Republic of Kazakhstan and the Kyrgyz Republic. Identifying gaps in the national regulation of telemedicine in these countries, the authors point out the aspects in which it is necessary to harmonize the regulatory framework for telemedicine consulting, and also provide for those provisions that must be necessarily disclosed in the agreement on telemedicine within the EAEU.
