Healthcare information systems and organizational model of care: current situation and opportunities for progress
The paper discusses possible improvement of healthcare through extension of its organizational model and better utilization of medical information systems. A brief review of ‘classical’ healthcare informatization tools is given with their relation to the steps of healthcare workflow that they intend to optimize. All consumers of healthcare are divided in four target audiences based on their needs in care and health preservation. Concepts of ‘health continuum’ and ‘continuum of care’ are discussed, with special focus on the parts of care continuum where informatization is currently lacking. The underlying reasons for such gaps are limitations of the prevailing healthcare organizational model. The current biomedical diagnosis-centric model of care is compared with biopsychosocial person-centric model.
Further progress of healthcare through wider application of biopsychosocial model should expand opportunities for informatization. -
Algorithms for solving problems of scheduling diagnostic and therapeutic measures in a medical information system
The statements and algorithms for solving the problem of scheduling medical examinations and treatment of patients
in a medical information system are considered. The problem of the optimal multi-day route for the provision of medical procedures to a patient is solved, taking into account the doctors’ work schedule. Cases of one- and multi-day service are considered. A description of the algorithms is given and their comparison is performed. -
Approaches to improving patient safety by means of MIS
Approaches to ensuring the safety of patient treatment in a medical organization are considered on the example
of surgical treatment. The proposed case is implemented on the basis of the JCI IPSG4 “Ensure Safe Surgery” standards and includes the implementation of preoperative verification and timeout by means of MIS. The interface is considered as a chain of interconnected checklists, which was named “Green wave of patient safety during surgical treatment”. -

Meanwhile, preparing a tender for IT services for Ministry of Defence depends on some specific peculiarities which are a subject of the current article – pricing, sole source purchasing, products from the Unified Register of Russian Software, etc. The article is intended to help healthcare provider specialists who conclude contracts on specified subjects to prepare and to make a procurement respecting interests of own company complying with the Russian law. -
Transformation of the role of healthcare information system. The growth of the role of HIS from automating the activities of an individual clinic to managing a large medical and prophylactic association
The article summarizes many years of experience in implementing medical information systems (MIS) in medical organizations. The article analyzes the transformation of the medical community’s ideas about the process of implementing MIS and its effectiveness for the period from 1994 to 2020. The features of the introduction of MIS in large medical and preventive associations are considered. There is a need to reorganize the business processes of a medical organization, including clinical processes.

Features of informatization of surgical departments of a medical organization. Planning and management of operational activities in a multidisciplinary hospital using MIS Interin
The article is devoted to the automation of the process of planning and management of operational activities in a
multidisciplinary hospital – the basic principles and features, parameters and factors affecting the formation of the operation plan are formulated, the model of the business process is described and criteria for its effectiveness are given, the methodological foundations for organizing the planning of operational activities are shown – shown the leading role of the relationship between the plan of operations and the plan of hospitalizations, the role of the medical information system in the automation of operational activities is determined.
All identified methods of increasing the efficiency of the operational activity of medical institutions by means of MIS were tested on the example of using MIS Interin PROMIS Alpha at the Federal Research and Clinical Center of the FMBA of Russia. The results that were achieved as a result of the practical application of these methods with the use of MIS in the organization of the daily process of the activities of surgeons in the Federal Research Center of the Federal Medical and Biological Agency of the Federal Medical and Biological Agency of Russia are considered. -
Patient-centered online platform as a service of digital healthcare ecosystem
Opportunities of healthcare informatization tools are closely related to and limited by requirements of the corresponding
model of care. The paper covers specific features of biopsychosocial patient-centered (BPC) model and their implications for informatization. Some approaches to automation of BPC model are discussed: ecosystem architecture and patient-centered online platform that provides communication infrastructure for consumers and providers of health services. Operation principles, functions and services of the platform are described that should help the platform meet the requirements of BPC model. The proposed online platform can supplement the existing healthcare informatization tools in addressing the challenge of chronic diseases epidemic. -

In the context of rapid technological progress, the outlines of the prospects for the development of telemedicine
technologies are becoming more and more clear. At the same time, changes in the organizational, legal and economic spheres often do not keep up with technological changes. This was especially evident in the conditions of the spread of the COVID‑19 coronavirus.
The article contains an analysis of the features of telemedicine technologies application in modern conditions, prospects and obstacles that arise in this area. -
Information exchange system for the implementation of special social payments to medical workers associated with the treatment of Covid-19 coronavirus infection
The government went to provide medical and other workers engaged in the treatment OF covid‑19 coronavirus
infection with a fairly large set of benefits, guarantees and compensation. Among the key and most financially intensive of them were incentive payments, which were replaced with special social payments from November 1, 2020.
At the same time, payments will now be made by the social insurance Fund. The implementation of such a scheme has caused the urgent need to create a complex system of information exchange, in which a large number of subjects participate, large amounts of information are verified and processed.
This article analyzes the emerging system of information exchange, as well as identifying possible risks and threats.
