  • 2017 № 2 Reference data for creating e-Health in Russia: problem overview.

    The article is about the importance of reference data (RD) at the e-health foundation, about the aims and structure of newly created Russian Ministry of Health (MOH) RD Control Center. The modules, services of the RD Support System have been described and key terms have been defined. Based on the experience authors formulate the development stages of references, and the unified reference structure (certificate) is introduced that defines common approaches for the reference development and support.

    The article section is dedicated to the tasks of the Object Identifier (OID) Control Center of Russian MOH objects. Principles of the OID Tree architecture built from the MOH root unique identificator are introduced. In conclusion, the authors state the most significant problems in the implementation of the Reference data System and propose ways of their solution.

    The article is devoted to the automation of the process of planning and management of operational activities in a
    multidisciplinary hospital – the basic principles and features, parameters and factors affecting the formation of the operation plan are formulated, the model of the business process is described and criteria for its effectiveness are given, the methodological foundations for organizing the planning of operational activities are shown – shown the leading role of the relationship between the plan of operations and the plan of hospitalizations, the role of the medical information system in the automation of operational activities is determined.
    All identified methods of increasing the efficiency of the operational activity of medical institutions by means of MIS were tested on the example of using MIS Interin PROMIS Alpha at the Federal Research and Clinical Center of the FMBA of Russia. The results that were achieved as a result of the practical application of these methods with the use of MIS in the organization of the daily process of the activities of surgeons in the Federal Research Center of the Federal Medical and Biological Agency of the Federal Medical and Biological Agency of Russia are considered.

  • 2020 № S5 Patient-centered online platform as a service of digital healthcare ecosystem

    Opportunities of healthcare informatization tools are closely related to and limited by requirements of the corresponding
    model of care. The paper covers specific features of biopsychosocial patient-centered (BPC) model and their implications for informatization. Some approaches to automation of BPC model are discussed: ecosystem architecture and patient-centered online platform that provides communication infrastructure for consumers and providers of health services. Operation principles, functions and services of the platform are described that should help the platform meet the requirements of BPC model. The proposed online platform can supplement the existing healthcare informatization tools in addressing the challenge of chronic diseases epidemic.

    One of the main objectives of the adoption of the Law of the Russian Federation of 29.11.10, No. 326-FZ «About compulsory medical insurance in the Russian Federation» is a significant improvement of the quality of medical care for Russian citizens. In accordance with the norm of Article 4 of the Law, the main principle of compulsory health insurance is to «...create conditions for ensuring the accessibility and quality of medical care provided under compulsory health insurance programs». Healthcare quality assessment can be implemented using the following methods:
    - medical standards compliance inspections;
    - pharmacotherapy effectiveness inspections;
    - medical and economical quality control;
    - questioning the quality of medical care, medical institutions rating, and medical staff rating.
    These methods allow to formulate the functional requirements for a healthcare quality information system. In practical implementation, such a system should contain a knowledge base on the basis of which it is possible to develop a formal scheme for the process of solving the tasks, as well as the implementation of the proposed knowledge representation model and the decision-making procedure

  • 2020 № 3 Improving the quality of dispensary medical examination of the population in an outpatient clinic based on the use of information technologies

    Introduction. A special feature of the adult population’s dispensary medical examination (DME) is the wide range of new tasks for the primary health care system. DME leads to an increase in the number of patients subject to dispensary follow up (DFU), an increase in the load on specialists of outpatient hospitals. Thus, it is necessary to search for and implement new tools in the practice of outpatient hospitals that improve the quality of preventive medicine and ensure that doctors make informed and
    timely decisions. Information technologies are offered as such a tool. Material and methods of study. This article uses methods of descriptive statistics and programming. The source material for the development of software modules was the materials of questionnaires and clinical and instrumental examination of patients, statistical reports on DME, DFU of patients in need at the district outpatient hospital from 2013 to 2018. The technical task and development of the algorithm for programming were performed by the authors. Comprehensive monitoring of risk factors for non-communicable diseases among those who have undergone DME was carried out using the program module “Screening of modifiable risk factors for major chronic non-communicable human diseases”, integrated with the MIS (medical information system of the institution). On this basis, the database “Monitoring of risk factors for chronic diseases among the population of the territory covered by the district outpatient hospital” was created. Improving the quality and increasing the coverage of the population with dispensary follow up was carried out using a software module developed for this purpose, integrated with the institution’s MIS. Results and discussion. A continuous comprehensive monitoring of non-communicated diseases risk factors has been organized, and their structure and changes over a 6-year period have been described. The proposed efficiency coefficient indicates positive changes in the work of the medical network to reduce the impact of risk factors on health. It was found that the most modifiable risk factors include “Low physical activity”, “Eating disorders”, and “Hyperglycemia”. DME leads to an increase in the number of patients who need DFU, an increase in the load on the primary health care network. The use of a software module in the practice of the institution to improve the quality and coverage of those in need of DFU contributes to positive changes in this section of work, facilitates the work of healthcare providers. Expanding the range of information technologies used in the work of outpatient hospitals contributes to improving the quality of decisions taken in the prevention of public health disorders by doctors working in conditions of increased workload. Conclusion. DME statistics indicate an increase in the burden on specialists of outpatient hospitals due to the increase in the detection of patients, the need to perform work to eliminate risk factors for non-communicable diseases, and an increase in the number of people subject to dispensary follow up. The work of specialists of outpatient hospitals to eliminate the impact of modifiable risk factors of non-communicable diseases should be carried out on the basis of their comprehensive monitoring. In these conditions, there is a need to expand the range of information technologies used in the preventive work of institutions providing primary health care, aimed at improving the quality of work in this section, creating new working conditions for healthcare providers. These results of using the proposed IT‑tools in integration with the operated MIS allow to improve the quality of preventive work in an outpatient facility, demonstrate the emergence of new tools for doctors that contribute for informed and timely medical decision-making.

  • 2016 № 5 Analytical processing of databases of the implemented information systems on the example of risk factors of socially dangerous actions of patients with organic damage of a brain.

    In article the possibility of use of the existing medical information systems introduced on the basis of medical institutions of the Tyumen region in studying of problems of public health is defined. On the basis of databases of the automated information system “Forensic-psychiatric Examination” the conclusions of out-patient forensic-psychiatric examination of the Tyumen region for 2005–2014 are analysed. The risk factors leading patients with organic damage of a brain to commission of socially dangerous actions are revealed. Are carried to such factors: a male, age till 30 years, the diagnosis “intellectual backwardness”. It is statistically proved that persons with this set of factors commit crimes under 159 and 161 articles of the Criminal code of the Russian Federation. The majority subexpert admit responsible a consequence. In article possible measures for primary prevention of socially dangerous actions of patients with organic damage of a brain are proposed.
