  • 2017 № 7 Comparative analysis and integrated assessment economic efficiency of the developed three-level system of provision of urological care to the population in the region compared to the traditional approach

    Medico-economic efficiency of functioning of the regional urological service at the modern stage of healthcare development is not optimal. In article the analysis of work-level system and the restructuring of urological care in Voronezh region for the total amount of direct costs for diagnosis and treatment of all patients and the cost of diagnosis and treatment in terms of a particular patient, for example urolithiasis, benign prostatic hyperplasia and prostate cancer. The study included two stages: a traditional system of provision of urological care to the implementation of the system restructuring, and after implementing three-tier system of care for patients with urological disorders. The results demonstrate a strong statistically significant objective reduction of economic costs of diagnosis and treatment for the vast majority of patients in all three nosological forms presented in this study reflect the ultimate performance of the regional urological service (on the example of Voronezh region), in terms of its functioning in the format developed a three-tiered system of medical care in comparison with the traditional approach.

  • 2013 № 1 Utilitarian analysis of the health care effectiveness in the group of patients with disabilities after coronary bypass surgery («Chelyabinsk State Medical Academy» of Federal Agency for Healthcare and Social Development the Russian Federation, Chelyabinsk, Russia)

    Summary. The research is based on the analysis of the register of patients operated on for CHD in the period 2000–2009 in Chelyabinsk interregional cardiac center on the basis of the Chelyabinsk Regional Clinical Hospital. Utilitarian analysis of cardiac surgery may play a crucial role in making organizational, clinical and financial decisions. The purpose — to carry out clinical and economic analysis with the use of the utility index («utility») DALY's (disability adjusted life years) in patients with coronary artery disease after coronary bypass surgery. The result — the values obtained allowed to conclude that a decrease in the degree of disability potentially duration of the life quality increases.

    The analysis of the reorganization of diagnostic service of a big Chelyabinsk private medical clinic by means of joining of a high-profitable ultrasound diagnostic department and endoscopic cabinets with low marginal profitability in the one whole. SWOT-analysis gave basis for pointing out inner and outer factors, influencing the diagnostic service development. As a result of two previously separated material development funds and introducing the system of complex examination of patients the material basis of the whole department was strengthened, that caused economic efficiency at its activity, analysed with the help of profitability markers: assets, production and selling.

  • 2022 № 1 Assessment of economic losses due to the development of infection in the surgical intervention area

    The development of infection in the field of surgical intervention is inextricably linked with financial costs and the public reputation of the institution. The medical and preventive institution needs to allocate additional funds from the hospital budget for diagnostic studies, as well as for medicines for the treatment of frolicking purulent-septic complications. As a result, medical and preventive institutions experience annual additional economic losses.
    P u r p o s e of the study is to assess the economic damage caused to medical and preventive institutions due to the development of infections in the field of surgical interventions.
    M a t e r i a l s a n d m e t h o d s . In the course of this study, an analysis of domestic and foreign literature sources on the issue of economic damage from purulent-septic complications in surgical practice was carried out.
    R e s u l t s . The damage caused by the development of infections in the area of surgical intervention is quite significant, and even the introduction of new technologies and therapies does not relieve patients from their possible appearance. The estimated costs of infection of the surgical intervention area vary depending on the region, the type of operation and the degree of infection.
    Economic analysis in this case is necessary to study the activities of organizations from the point of view of evaluating their work in order to monitor and subsequently better manage the processes of supplying the institution itself with material resources, as well as the use of internal reserves and their rational use.
    F i n d i n g s . The distribution of funds in medical and preventive facilities should be carried out rationally to ensure the full-fledged activity of the institution. In the absence of measures taken to prevent the development of nosocomial infections, infections of the surgical intervention area may occur after a certain period of time after the patient is discharged from the hospital, thereby contributing to the spread and contamination of healthy people, which from a socio-economic point of view is unprofitable.
