Staff time specialist doctors for the provision of medical care in the outpatient setting
In order to implement the Decree of the President of the Russian Federation dated May 7, 2012 № 598 «On improvement of state policy in the sphere of public health» and in accordance with the Action Plan of the Ministry of Health Labor norms in health care, approved by the Interagency Working Group on the organization of rationing of work, the development of professional standards and skill levels in the health sector conducted observed for 2 working weeks of the working process of two specialists: cardiologist, endocrinologist and dentist-therapist in 20 pilot areas of the country. Data analysis observation revealed the need for regulations to establish standards load specialists providing outpatient care to the population due to an increase in the time to visit a patient.

The economical effectiveness of actions preventing the expansion of incidents of substance and alcohol abuse
The analysis of economical effectiveness of preventative measures against substance and alcohol abuse in the Kostromskaya region between 2008–2012 years showed a presence of significant losses for the region’s economy, prompted by the population’s morbidity for drug and alcohol abuse among the young and employable population. There was noted a low level of financial provision of regional target-oriented programmes and a deficit in current financing of narcology service, what in turn makes the medical aid considerably less accessible. The level of achieved indicators for evaluating the effectiveness of the narcology service’s activity demonstrated that there is a significant retardment among the regions, included in the process of realization of the concept for modernizing the narcology service of the Russian Federation.

Needs of the population Nizhny Novgorod region in cardiac surgery procedures
The article presents the analysis of security of the population of Nizhny Novgorod region with medical staff and bed capacity cardiology; analyzed actually existing cardiac surgery volumes in the Nizhny Novgorod region; designed neediness of the adult population of the Nizhny Novgorod region in cardiac surgery based on data from a multicenter epidemiological study СТЕРХ.

Management of blood donor: platelet pooling
Based on the cost analysis of different types of platelet concentrates (PC) assessed the effectiveness of the introduction of technology pooling of platelets in the Irkutsk Regional Blood Transfusion. Comparative cost analysis of different types of PC, the cost of consumables for the same therapeutic dose PC was performed. It was found that the content of pooled PC cells does not give in to apheresis. The introduction of technology pooling of platelets allows significantly (by 54,4% or more) to reduce the cost of issuing PC. To evaluate provision of PC in the regions it is advisable to consider the number of harvested PC through methods for preform at therapeutic doses.

Analysis of the official regulation documents on organization of outpatient care for patients with chronic complications of diabetes mellitus
Diabetes mellitus (DM) is a socially important chronic disease with the disabling and economically significant complications. Organization of specialized structures for interdisciplinary out-patient care for patients with chronic complications of diabetes (affecting kidneys, eyes, the lower extremities) is an important task. We analyzed existing National clinical expert recommendations and orders of Ministry of Health concerning standards of care for patients with DM and its complications. Conclusions are drawn on necessary development of the regulatory documentary base for out-patient multidisciplinary care for this kind of patients.

Structure and challenges of the medical division of the new programme of fundamental scientific research of state science academies
The article considers the questions of planning and financing the scientific-research activity in the field of medical science, realized at the cost of the state budget. Authors demonstrate that disintegration of scientific processes leads to a decrease of intensity of scientific search and implementation of achievements in medical sciences in the health care practice. Authors conclude that it is necessary to increase the effectiveness of the management of scientific research and more clearly define the priorities of the medical sciences development.

The challenges, related to provision of paid medical services on anonymous basis
Current legislation has quite clearly settled the questions regarding the provision of the medical aid on a paid basis. Medical organisations, participating in the realization of the programmes of state guarantees for delivering free medical aid and territorial programmes of state guarantees for providing free medical aid to the population have a right to provide patients with paid medical services anonymously exempt for the cases set forth by Russian legislation. However, the provision of medical aid on a paid basis requires conclusion of contracts. There is no special legislation or other normative regulation for contract conclusion on anonymous basis. Nevertheless, an analysis of the normative basis, related to the anonymous provision of medical aid allows finding solutions to the most frequently occurred problems.
