  • 2017 № 9 Problems of rendering paid medical services from the position of heads of medical institutions

    In accordance with the legislation in the field of health care, every citizen of the Russian Federation has the right to receive medical care, which is implemented by providing a guaranteed volume of free medical care and the possibility of obtaining paid medical services. Despite the legally established procedure and conditions for the provi- sion of paid medical services, medical institutions of state and non-state forms of ownership have a large number of problems when providing paid medical services. The object of this study was an expert evaluation of the organization of paid medical services at the level of the administrative center of the subject of the Russian Federation. The experts were the manager of medical institutions that have a certificate on the specialty «Public Health and Health Care». To eliminate the random nature of the comments, a study was conducted to determine the homogeneity of the sample using the nonparametric Cochran criterion. The results confirmed the existence of problems in the organization of paid medical services and allowed to identify the most significant problem areas

  • 2022 № 1 Portrait of regional health authority manager

    Competent management is crucial condition for the development and functioning and requires qualified managers. In the Russian Federation over the past few years Federation there were many rotations in management of regional health authorities. These changes will be followed by lack of continuity in management decision-making, high degree of uncertainty and a low level of population and professional community trust.
    T h e a i m o f t h e s t u d y was to give a detailed information about regional health authorities managers in Russian Federation.
    M a t e r i a l s a n d m e t h o d s . This research was designed as observational study with the use of questionnaire which included the characteristics of the age and gender of the respondents, the level of their basic and additional education, additional competencies, occupational background in leadership position.
    R e s u l t s . We achieved a response from 81 respondents. We founded that rotation of managers and extremely short occupation period had significant impact on managers’ qualifications. The culture of purposeful formation of the professional skills by the respondents was poor. Only 7% of managers paid attention to their education. About 40% did not have professional retraining for management in healthcare.
    C o n c l u s i o n s . It is necessary to develop a standard set of Russian region health managers competencies and responsibilities.

  • 2023 № 3 Portrait of regional health authority manager – 2022

    The heads of regional executive authorities in the sphere of health protection are key figures in the realization of the Russian Federation’s healthcare development strategy. Without qualified managers, it is difficult to achieve national goals and objectives of federal projects. The study of the qualifications, professional experience, and skills of the high leaders of regional health systems allows the evaluation of top personnel’s human potential and the successful implementation of steps to increase the efficiency of its administration.
    T h e a i m o f t h e s t u d y was to give a detailed information about regional health authorities managers in Russian Federation.
    M a t e r i a l s a n d m e t h o d s . A single-stage, continuous sociological study was planned and carried out using a specially developed questionnaire, which included a characterization of the accounting characteristics of participants’ age and demographic characteristic, level of basic and additional education, availability of professional competencies, and work experience in managerial positions.
    R e s u l t s . We achieved a response from 85 respondents. It was found that, as compared to 2021, there was a slowdown inside the process of rotation of regional ministers’ personnel and an increase in the degree of professional qualifications of managers in 2022.
    These trends were reflected in the increase in the average length of service in the position of regional minister (2021 2,1±1,9 years; 2022 2,6±2,5 years), the doubling of the number of managers with work experience of up to one year (2021 n = 42; 2022 n = 21), and the decrease in the number of managers with work experience of up to one year (2021 n = 42; 2022 n = 21).
    C o n c l u s i o n . The research allows for an assessment of the dynamics of changes in regional health care systems in terms of managerial human resources from 2021 to 2022. Positive declining trends in 2022 have been identified. The pace of rotation of regional ministers’ staff adds to the development of leaders’ professional abilities and experience and, eventually, will be the key to accomplishing strategic goals in health care.

  • 2019 № 5 Satisfaction of the population with the organization of clinical examination of certain groups of the adult population on the example of a constituent entity of the Russian Federation

    One of the criteria for the quality and availability of clinical examinations is the satisfaction of the population with the obtained preventive care. The subject of the study was the level of satisfaction of citizens who underwent medical examination, accessibility and quality of its organization. The following methods are used in the work: content analysis of scientific literature and regulatory documents; statistical method; analytical method. The reliability of the results obtained was evaluated by the method of calculating confidence limits. Applied correlation analysis. A sociological survey of 568 respondents who have undergone clinical examination of the adult population. Identified a number of subjective and objective problems in the organization of clinical examination. The degree of satisfaction of the population with the availability and quality of the organization of clinical examination of the adult population is determined. A correlation was found between the satisfaction of citizens with clinical examination and its availability. Proposed measures aimed at improving the organization of medical examination.

  • 2019 № 2 Influence of socio-economic living standards of the population of the region on the market of paid medical services

    The purpose of the study was a comprehensive analysis of the market of paid medical services in the Trans-Baikal Territory, taking into account the influence of regional factors. Using the correlation analysis, the influence of regional socio-economic factors on the change in the volume of paid medical services has been studied. To assess the market of paid medical services, a comprehensive analysis of indicators characterizing consumers and service providers on a reimbursable basis was carried out. Analysis of the activities of medical organizations that provide paid services in the framework of primary health care reflects an increase in the number of visits per shift, specialized medical care – an increase in the number of beds. These data indicate an increase in demand for paid medical services among the population of the region and an increase in their volumes. A demographic estimate of the population of the Trans-Baikal Territory as consumers of paid medical services has revealed a decline in the economically active population, the over-mortality of able-bodied citizens, and the depopulation of the entire population in the region. Socio-economic analysis identified a slight increase in gross regional product, an increase in average per capita incomes, as well as an increase in unemployment, a decrease in the number of employees, etc. Among the factors affecting changes in the volume of paid medical services, indicators characterizing the socioeconomic standard of living of the population were identified. The study made it possible to characterize the market for paid medical services from the perspective of producers and consumers of services, as well as to identify the influence of regional socio-economic factors on the volume of paid medical services.
