  • 2023 № 4 Aspects of the quality of the life of persons of working age with chronic noncommunicable diseases of the respiratory organs

    Chronic noncommunicable diseases (NCD) are widespread and affect people of different age groups in all countries. The fight against this group of diseases is one of the priorities of the modern health care. Non-infectious diseases of the respiratory system as well as other diseases, affect social, physical and mental components of well-being. The qualitative assessment of their influence helps to assess the study of the quality of life.
    P u r p o s e : to assess the quality of life of the working-age population of the Nizhny Novgorod region and to develop measures to reduce the risk factors for chronic non-infectious diseases of the respiratory system in these cohorts.
    M a t e r i a l s a n d m e t h o d s . A one-stage comparative study was carried out using the method of standardized interviewing using the SF‑36 questionnaire. The quality of life of people of various age-gender groups with chronic non-infectious diseases of the respiratory system was assessed. All respondents were stratified into groups by pathology and age.
    R e s u l t s . The scientific article reflects the results of the study of assessing the quality of life of working-age patients with the chronic respiratory system pathology. The analysis of the data revealed statistically significant differences in age, gender and groups of pathologies among the respondents. Differences in indicators reflecting emotional states were revealed among men and women of the working age. Age-related changes in indicators of physical and psychological components of health and on all scales of the quality of life were presented. The obtained values of patient indicators show the need to develop specific measures to reduce behavioral risk factors for chronic non-infectious respiratory diseases of the population of the Nizhny Novgorod region.
    Findings. The comparative analysis of the results of the scales of the quality of life and health components reflects the age-related changes in indicators. The people of the older age group of the working age demonstrate significantly low results. The indicators of physical functioning are statistically significantly different in different pathologies of the respiratory system. Chronic obstructive pulmonary disease (COPD) restricts the physical activity of patients more strongly than other pathologies. Low values of the quality of life reflect the need to develop the measures to reduce the risk factors for chronic non-infectious diseases for the population of the Nizhny Novgorod region.

  • 2020 № 8 The core vital values and requirements defining quality of life of country people are more senior than working-age of the Kaliningrad region

    Article is devoted to studying specific needs of persons also their interests affecting quality of their life are more senior than working-age. The purpose of work is to reveal the core vital values and requirements defining quality of life of country people is more senior than working-age of the Kaliningrad region. Within the project of Partnership “Northern measurement” in the field of health care and social wellbeing in 2019 the social research among 211 villagers aged from 60 up to 88 years is conducted. Methods of carrying out work: sociological, analytical, mathematical statistics. Distribution of age and gender structure of the interviewed population reflects universal trends. Social and demographic and medico-social characteristics of country people are studied. Results of a research showed that the quality of life of persons is more senior than working-age most is defined by priority vital values and requirements: state of health and level of material welfare; loneliness; difficulties in use of digital technologies; the disrespect shown from youth i.e. quite objective reasons which solution generally depends on the state measures for support of the senior generation. Significant first 10 vital values at elderly people during the different periods of time are tracked.

  • 2021 № 9 Borderline functional states of the men’s reproductive system: its influence on health and the quality of life.

    Between the differing frequency of prevalence of sexual dysfunctions, there’s a particular interest in the borderline disorders of the male reproductive system, in which no obvious somatic and mental pathology can be found in the body. The reversibility of functional disorders unveils favorable prognostic prospects and presupposes the development of new approaches in the organization of observation and correction of minimal borderline functional states of the reproductive system.
    P u r p o s e . To identify the frequency of borderline functional states of the reproductive system (BFSDS) among men over a five-year dynamics, their impact on the formation of health outcomes and quality of life.
    M a t e r i a l s a n d m e t h o d s . The study was conducted at the Reproduction Center of the Clinical Diagnostic Center (Nizhny Novgorod) for five years. Two groups of men of reproductive age with BFSDS at the age from 20 to 60 years were tested. We used copy-pair method. The first group (DM) included men with sexual dysfunctions and similar sexual disharmony (BFSDS) who didn’t receive medical benefits. The second group (DM + treatment) consisted of men with BFSDS who received cognitive-behavioral therapy and medicine. All subjects assessed the state of the reproductive system, neurological and hormonal status, as well as the quality of life (according to the SF‑36 questionnaire). To compare the parameters of the quality of life, a control group was selected – healthy men of reproductive age.
    R e s u l t s . The frequency of sexual dysfunctions in both groups is significantly increasing, but the frequency of their detection in men receiving treatment is 2,4 times lower than in the other group. An increase in neurological changes and hormonal disorders was recorded, the prevalence of which depends on the duration of observation and the availability of therapeutic measures. The study proved that the BFSDS have a significant impact on the quality of life of men, especially in the physical and psychological components of health. It was shown that the treatment allowed the patients to preserve the state of physical and, to a greater extent, psychological comfort during the five-year observation period.
    I n c o n c l u s i o n , the authors emphasize the need for new approaches to the organization of observation and correction of the minor BFSDS.

  • 2022 № 6 Peculiarities of the quality of life of students, including with disturbances in physical development, under the conditions of the COVID‑19 pandemic

    In the context of the COVID‑19 pandemic, student youth are facing significant adverse impacts that affect the state of physical and mental health, as well as changes in quality of life indicators. Factors such as the sudden transition of students to online learning, the closure of hostels, limited access to public spaces, an increase in morbidity in the close environment, the manifestation of fears and phobias had a negative impact. Students have irregular meals, limited access to good nutrition, increased time spent in front of the monitor, less opportunities for physical activity and associated significant changes in physical parameters, namely obesity, an increase in body mass index weight.
    Materials and methods: bibliographic, socio-hygienic, sociological, statistical, analytical methods were used in the work.
    Findings. Analysis of the results of the study made it possible to establish: a) the average level of the physical component of students’ health is higher than the psychological one; the indicator of the physical component of health, corresponding to the norm, was observed in 70.7% of students; psychological – in 63.4% of students; no statistically significant relationship was found between the physical and psychological components of students’ health; b) in terms of the physical and psychological components of health, the largest percentage of respondents with a norm was noted among students with a lack of body weight; a low level of the physical component of health is much more common in the group of students with excess body weight, compared with the other studied groups

  • 2022 № 8 Prediction of disease outcomes in patients with COVID-19 based on the EQ 5D 5L questionnaire

    In order to provide effective medical care to patients with COVID-19 in hospitals, the disease outcome prognosis is of
    considerable importance. The authors of the study suggested a relationship between health-related quality of life indicators and the outcomes of the disease of patients hospitalized because of COVID-19.
    P u r p o s e . Retrospective study of the relationship between health-related quality of life in patients with COVID-19 on the first day of hospitalization with the outcomes of the disease in a hospital and at the stages of rehabilitation up to 180 days following the discharge to predict outcomes at the early stages of the disease.
    M a t e r i a l s a n d m e t h o d s . The study was conducted at the “Kommunarka” Moscow Multidisciplinary Clinical Center of the Moscow City Health Department from March 2021 to April 2022. Data collection was carried out using a paper-and-pencil Russian version of the EQ-5D-5L questionnaire (Tracking Number: 41183), on the first day of admission of patients to the hospital. Additional patient data was extracted from electronic health records. Two types of EQ-5D-5L questionnaire results were evaluated: Index EQ and the Visual Analog scale (VAS) EQ. All calculations of the regression model were carried out using the R 4.2.1 language in the RStudio 2022.02.3 Build 492 software environment.
    F i n d i n g s . In the obtained logistic regression model, the extremely high statistical significance of the variable “Index EQ” (p<0.01) was demonstrated, the greater the value of the variable “Index EQ” is, the less the probability of a fatal outcome is.
    At the same time, there is no statistically significant relationship between the variable VAS EQ under study and the probability of a fatal outcome.
    C o n c l u s i o n s . An extremely significant relationship between the calculated health-related quality of life index, Index EQ, and outcomes of COVID-19 when patients were admitted to the hospital was revealed, which makes it possible to use it as a predictor of disease outcomes at early stages. No significant relationship between the VAS EQ and the outcomes of the disease in patients with COVID-19 on the first day of hospitalization was revealed, which makes it impossible to use it as a predictor of disease outcomes at the early stages. Application of the health-related quality of life index, Index EQ can be used to make medical decisions in order to early predict tactics of patient management in hospital treatment and rehabilitation.

  • 2023 № 7 Review of the regulatory framework regarding the use of multifocal IOLs for cataract surgery.

    Considering the increasing requirements for the quality of postoperative vision, the number of requests from patients for the implantation of a multifocal IOL is increasing, which makes it relevant to analyze the existing regulatory framework for the possibility and obligation to use this medical device in the provision of specialized medical care – surgical treatment of cataract within the framework of the state guarantees program. An analysis of the current regulatory framework showed that the existing regulatory framework allows the use of multifocal IOLs in the provision of medical services under the state guarantees program, however, it does not give the patient the right to choose, and makes considering the patient’s desire to implant this particular type of medical device is not mandatory when planning
    surgery. Considering the requirements of mandatory informed voluntary consent when receiving a medical service, the patient must be informed about the implantation or refusal to implant this type of medical device when providing him with specialized medical care. Most of the analyzed medical devices – IOLs with multifocal properties – are registered in the Russian Federation as “Intraocular lens for the posterior chamber of the eye, pseudophakic”, and not “Intraocular lens for the posterior chamber of the eye, pseudophakic, with increased depth of focus”. There is both a clinical need for changes (Clinical guidelines), and an organizational need to identify a group of operations, including with the application of additional coefficients to the tariff as part of the payment for medical services included in the program of state guarantees.

  • 2021 № 8 Study of quality of life related to health in patients with COVID19 on the first day of admission to hospital

    The COVID‑19 pandemic has resulted in exceptional social disruption and consequent changes in quality of life. Assessment of the health-related quality of life in patients infected with the SARS-CoV‑2 (2019-nCoV) virus in a covid hospital is an urgent public health issue that affects the organization of medical care processes and the planning of rehabilitation programs.
    Purpose of the study is to determine the level of health-related quality of life in patients with COVID‑19 on the first day of inpatient treatment for comparison with pre-pandemic norms and planning targeted rehabilitation programs.
    Materials and methods. A study of the quality of life was carried out using the EuroQol EQ‑5D‑5L questionnaire assessing the Index EQ and EQ VAS indicators. Using the method of simple random sampling, the data of patients were collected for three sets having following features: “Both sexes”, “Men”, “Women”. The analysis was carried out using descriptive statistics methods.
    Results. The indicators of health-related quality of life in patients with COVID‑19 on the first day after admission are lower than the pre-pandemic population norms for Moscow residents according to Index EQ by 20%, according to EQ VAS by 29%, on average. Women have more health limitations than men in all EQ‑5D‑5L domains, which is proportionally lower than population norms. In 3,7% of patients on admission to the hospital, the type of quality of life corresponded to “a condition worse than death”, which was practically not observed in the pre-pandemic period in Moscow residents. The greatest losses are observed in the “Self-Care” and “Mobility” domains, both for men and women. On admission to the hospital, no differences were found in the pre-pandemic and pandemic levels of health limitations in the “Anxiety/Depression” domain.
    Conclusions. The results of the study can be used by specialists from various branches to plan clinical, service, educational, economic, and other public health programs related to the rehabilitation of patients who have had COVID‑19. And it also represents an indispensable additional material for further sociological and clinical research.
