  • 2021 № 5 Role of the “national medical chamber” in coordinating the activities of medical professional communities (for the anniversary of the “National medical chamber”)

    The field of healthcare in the modern period of its development is so multifaceted, complex, and also due to the synthesis of professional competencies and skills from various fields of medical science and jurisprudence, which requires a pooling of efforts for its effective functioning.
    Purpose of the study is to analyze the current activities of the National Medical Chamber as the main aggregator of medical professional communities and medical associations.
    M a t e r i a l s a n d m e t h o d s : materials for the undertaken analysis were literature sources, documents, resolutions. Methods of comparative analysis and systematization of information, content analysis were used in the work.
    R e s u l t s . The article provides an analysis of the activities of the “National Medical Chamber”, created ten years ago with the aim of aggregating medical communities in their professional activities towards improving the health care system of the country’s population. It is shown that these trend directions have become key, meeting the modern needs of the effective development of the industry in conjunction with the medical professional communities, the Ministry of Health of the Russian Federation, and world achievements. The article reflects the issues of preserving the problems of self-government in the activities of various medical professional communities and medical associations.
    F i n d i n g s . The ten-year period of activity of the “National Medical Chamber”, based on a consistent, reasoned, analytically verified ideology, allows us to calibrate and clearly solve the assigned tasks on the “painful areas” of healthcare.

  • 2020 № 1 Health care Financing in 2020: priorities within the national project «Health Care»

    Analysis of future health financing shows that in 2020–2022, despite the difficult financial situation, the growth of financial support for the health system will continue. Among the priorities for 2020, we should highlight the increase in funding for cancer care. This is the implementation of the provisions of the Decree of the President of the Russian Federation on may 7, 2018 No. 204 "on national goals and strategic objectives of the development of the Russian Federation for the period up to 2024". The allocation of funds in the compulsory health insurance system is also based on these priorities. In particular, at the expense of mandatory medical insurance funds, financial incentives are provided for employees to identify oncological disease. Among the positive aspects are inter-budget transfers to the budget of the compulsory health insur¬ance Fund from the Federal budget to pay for high-tech medical care that is not included in the basic program mandatory medical insurance and birth certificates. All this should have a positive impact on the further development of health care.

  • 2021 № 1 Financing of healthcare in 2021 from the federal budget: priorities within the national project “Healthcare” in the context of the fight against coronavirus infection Covid‑19

    An analysis of the upcoming financing of healthcare from the Federal budget shows that in 2021–2022,
    despite the difficult financial situation, the volume of financial support for the industry will continue to grow. These are all
    the more important parameters because the upcoming budget in the period under review assumed a slight reduction in
    funding compared to the approved figures of the Federal budget for 2020. This should help to ensure the stable functioning
    of health care.

  • 2022 № 5 Ten years of law enforcement of Federal Law No. 323-FZ dated 21.11.2011 “On the basics of public health protection in the Russian Federation”: controversial issues, gaps, prospects

    Federal Law No. 323-FZ of 21.11.2021 “On the Fundamentals of Public Health Protection in the Russian Federation”, adopted in 2011 and entered into force in 2012 (323-FZ) is a key federal law regulating the healthcare sector.
    The authors, through content and retrospective analysis, deductive method, method of generalization of judicial practice, conducted a comprehensive study of the application at the 10–year turn of the basic law in the healthcare system of the Russian Federation – 323- FZ. In the article, the authors analyzed the subject of legal regulation of the 323-FZ, the conceptual apparatus enshrined in 323-FZ, the principles of health protection, considered the ratio of the legal statuses of the patient and the medical worker, touched upon aspects of paid medical services. The problems in the implementation of the 323-FZ are also analyzed, the existing legal uncertainties of its enforcement are discussed.
    According to the results of the study, the authors conclude that it is necessary to consolidate the most complete guarantees of the realization of rights, both for patients and for medical workers, aimed, inter alia, at reducing the risks of conflicts between the patient and the doctor, regulating economic relations in health care. In this connection, it is necessary to carry out work on the timely completion of this law.

  • 2016 № 1 The paradoxes of health care funding in 2016: the system of mandatory health care insurance policies recorded extra funds, which will be withdrawn back into the federal budget

    With all complexity of current economic landscape in the country, health care remains to be one of the top priority spheres regarding financing. Despite the crisis, the volume of funds, allocated to the field is gradually increasing. However, the growth rates of spending on health care lag behind the rates of inflation. Furthermore, a significant part of the budget of Federal Fund of the mandatory medical insurance policy is not connected with the provision of medical aid in the medical mandatory insurance system. Expenditure of Mandatory Medical Insurance System is not oriented on President’s orders to increase the salaries. And despite the presence of crucial financial issues in the mandatory medical insurance system, substantial funds are withdrawn from the Federal budget of Mandatory Medical Insurance system into the federal budget.

  • 2023 № 7 Organization of the educational process in improving the primary prevention of non-medical consumption of psychoactive substances by minors.

    The article presents the results of an experiment conducted in teaching teenagers from among healthy individuals and at-risk groups to prevent the use of psychoactive and other non-drug drugs using a differentiated approach taking into account their metacognitive styles and information coding styles. The main goal of teaching teenagers children was to instill motivation for health-saving behavior as the main meaning of life. After the classes conducted by specially trained personnel from among teachers, narcologists, medical psychologists, nurses with specialized teenagers, positive results were noted in increasing the assessments of hygienic competence of teenagers, their
    medical activity and motivation to preserve.

    This science literature review was conducted for five-year period. It has been established that the process of improvement by lean management has more than 50 years history in industry, and the experience is best carried out, in primary health care; the main of lean management introduction in healthcare is to ensure optimal participation of medical staff in the changes; the concept of medical care in the first place has the aim of improving the quality and availability of healthcare processes; mapping is a basic tool allows to visualize the process of changes and evaluate progress; project management is a method of comprehensive changes in healthcare in which the elements of lean management are also used to ensure general increase in the efficiency of transformations. Lean management is becoming increasingly scientific and practical tool in health management nowadays.

    Healthcare in developed countries becomes the most productive and cited field of the world science. However little is known about innovative processes in the non-market oriented areas including medicine. This article deals with funding and managing research and development in medicine in foreign countries. The authors show that innovations in healthcare are the major focus of interest of foreign governments seeking for balance between cost-containment, quality and access to health care. Healthcare innovations assessment is necessarily accompanied with the expert evaluation of efficacy and safety of health technologies delivered by designated institutions. International policy analysis confirms that the search for the most effective management in organization and funding of healthcare research is of high priority in the world.

  • 2023 № 6 Experience in the application of artificial intelligence technologies for the development of preventive health care on the example of the Kirov region.

    Healthcare is one of the priority sectors for the practical application of artificial intelligence (AI) systems. In 2018, in the Kirov region, it was decided to launch its own regional project for the implementation of AI technologies in order to gain practical experience and understand the features, advantages and barriers to the use of AI. Improvement of preventive medicine was chosen as a priority direction.
    The Russian predictive analytics platform Webiomed was chosen as the base software product. The project implementation included 3 stages: pilot testing in 2019–2020, commercial operation in the “second opinion” mode in 2021–2022. and implementation in the digital assistant mode, launched in 2023. As a result of the implementation of the 1st and 2nd stages of the project, it was possible to prove that the main advantage of AI in the analysis of big medical data is the autonomous and high accuracy of interpretation of the information available in it. An AI system for independently extracting the data necessary for analysis from electronic medical records, comparing them with data from past periods, assessing the dynamics of changes in health indicators, and identifying the emergence of dangerous trends and risk factors. Together, this allows the formation of the so-called “digital profiles” of patients, which in turn are a valuable resource for supporting management and clinical decision-making.
