Article devoted to the new aspects of the development of medical information systems (MIS). The article will be useful
for both architects and developers MIS and users of modern MIS.
Hospital replacing technologies in the digital ecosystem of health management
HIS multicomponent mechanism: areas of application
The Article is devoted to discuss the conditions and variants of using the multicomponent mechanism in HIS.
The KPI application for Healthcare Information System`s project
The article describes approaches to using key performance indicators for healthcare institutions computerization and its problems (contracts for projects of healthcare information system (HIS) development for healthcare institutions, its advancing or HIS support). The article provides examples of real indicators defining project success, KPI‑based project estimating recommendations given. The article will be useful for healthcare institutions specialists preparing such contracts and for such contractor project managers.
Informatization features of curative pedagogics organization
An approach of informatization of curative pedagogics organization is proposed. Features of curative pedagogics organization and its business-processes is described. An approach for creation of Curative pedagogics information system (CPIS) is formulated, the structure and functional requirements CPIS are described, perspectives of future development CPISare outlined.
Tasks of distribution of medical appointmen
The statements and algorithms for solving problems on the formation of routes for medical examinations and treatment are discussed. The problems of the optimal route for an individual patient are considered taking into account the doctors’ work schedule and the distribution of appointments for a group of patients with the same and different medical care requirements
Rapid prototyping of graphical representations of data (dashboards) for the head workstation in the HIS
The article is devoted to the methods of rapid prototyping of graphical representations of data for monitoring the performance of the management object in the functioning of medical organizations formed with the use of tools Oracle Apex.
Prediction of recurrence in patients with Cushing’s disease after successful endoscopic transnasal adenomectomy: neural network model and its software implementation
Introduction. Due to the high frequency of recurrences in patients with Cushing’s disease after endoscopic transnasal adenomectomy (up to 55% in the 5 year period), it is important to develop a method for predicting recurrence of the disease based on a combination of factors. Мaterials and methods. The study included 219 patients who underwent endoscopic transnasal adenomectomy in 2007–2014.Over 3 years, remission persisted in 172 patients; relapse developed in 47 patients. The construction of artificial neural networks of various topologies was performed in the Statistica v. 13, and then software was developed for the best network.
Results. A highly efficient neural network (3-layer perceptron) was constructed, which allows predicting recurrence within 3 years or remission for at least 3 years. The sensitivity of the model is 74%, the specificity 97%, the positive predictive value 85%, the negative predictive value 93%. The predictors of the model are sex, age, duration of the disease, MRI type of adenoma, levels of adrenocorticotropic hormone and cortisol in blood in early postoperative period. Web-calculator was developed and is available to doctors for free practical use on http://medcalc.appspot.com/.
Сonclusion. The software implementing neural network is a quite effective tool for predicting recurrence and it will allow to perform personalized approach to management of patients who underwent neurosurgical treatment for the Cushing’s disease.
Knowledge-based diagnostic system for orphan diseases development
The paper discusses the problems of early diagnosis of rare hereditary origin diseases in children, presents a brief description of previously created systems aimed at providing support for physician assistance in diagnosis. The prototype of an expert diagnostic system is presented, the knowledge which combines bibliographic data on clinical cases, the coefficients of modality of signs (based on expert evaluations) and certainty factors for manifestation and severity of signs over four age periods. Such an approach to the formation of a knowledge base allows improving the quality of differential diagnosis of a rare pathology at an early age in order to start treatment on time to prevent the development of pathological manifestations caused by the accumulation of macromolecules in organs and tissues.
